Cancer de la prostate 2010. Innovations thérapeutiques

Résumé le congrès de la Société française de radiothérapie oncologique (SFRO), un symposium, dont le thème était « Les innovations thérapeutiques », a mis en avant quelques questions très discutées dans la prise en charge du cancer de la prostate. Quelle est la meilleure technique d’augmentation de la dose : curiethérapie ou radiothérapie avec modulation d’intensité ? Des arguments existent pour chacune des options sans qu’actuellement, en l’absence de réel essai comparant les deux techniques, nous ne puissions trancher. L’intérêt de l’hormonothérapie néo-adjuvante, avant chirurgie ou radiothérapie a également été discuté, ce d’autant que l’arrivée des antagonistes de la LH-RH permet maintenant d’avoir une action rapide et profonde. Enfin, les traitements de rattrapage pour rechute biochimique après irradiation externe ont été évoqués : chirurgie, cryothérapie ou ultra-sons focalisés, ainsi que l’heure de l’hormonothérapie. the annual congress of the SFRO, a symposium, on “Therapeutic innovations”, described some of the burning questions in the treatment of prostate cancer. What is the best technique to perform radiation dose escalation, brachytherapy or Intensity modulated radiotherapy ? Rational has been highlighted for both techniques, but without a prospective comparative trial, no definitive answer could be given today. The role of neo-adjuvant hormonal treatment, before surgery or radiotherapy is still discussed, particularly in the area of LH-RH antagonists which have a rapid and profound anti-hormonal effect. At least, the salvage treatment for biochemical relapse after external beam irradiation has been discussed: surgery, cryotherapy, and High-Intensity focalised ultrasons and the timing of hormonal treatment.
