  • Abstract
    Purpoe: To evaluate the afety and efficacy of extracapular cataract extraction with poterior chamber intraocular len (ECCE/PC-IOL) compared with intracapular cataract extraction with aphakic glae (ICCE-AG). Method: In a nonmaked randomized controlled clinical trial, 3,400 bilaterally viionimpaired patient aged 40 to 75 year with operable cataract were randomly aigned to receive either ICCE-AG or ECCE/PC-IOL at the Aravind Eye Hopital in India. The urgery wa performed by one of four tudy urgeon. Patient were hopitalized for 5 potoperative day, with followup viit at 2, 6, and 12 month after dicharge. Poturgery evaluation were conducted by two independent tudy ophthalmologit. Reult: At any ingle potoperative follow-up time point, there were no tatitically ignificant difference of clinical relevance between treatment group for any complication of a eriou nature except cytoid macular edema, which wa more common with ICCE (4.2% v 1.6%). In general, whether of a trivial, intermediate, or eriou nature, complication rate were low at each evaluation time point. Cumulatively, the incidence of eriou complication of all type throughout the 1-year tudy period wa 14.5% for patient in the ICCE-AG group and 7.7% in the ECCE group (P < .001). Bet-corrected viual acuity of 20/40 or better at 12 month wa attained by 90.7% of ICCE-AG patient and 96.3% of ECCE/PC-IOL patient (P < .001). Concluion: Although both operative procedure are afe and effective for cataract patient with bilateral impairment, ECCE/PC-IOL i uperior to ICCE-AG in term of both viual acuity retoration and afety.
