  • Abstract
    PURPOE: To report a cae of peritent polymicrobial potoperative endophthalmiti caued by Alcaligene xylooxidan and Propionibacterium acne in a peudophakic eye. A. xylooxidan i a gram-negative bacteria reitant to mot antibiotic. METHOD: Cae report. REULT: A 72-year-old man preented with clinical ign of endophthalmiti on the firt potoperative day after a phacoemulification procedure with poterior chamber intraocular len, left eye. Initial treatment included topical, ubconjunctival, and oral antibiotic. After initial clearing, there wa recrudecence of infection on potoperative day 37 that prompted referral of the patient to the Cullen Eye Intitute. Treatment at that time included anterior chamber and vitreou tap with intravitreal antibiotic injection. Complete par plana vitrectomy and intraocular len explantation were eventually required becaue of peritent infection with a reitant organim. Culture from the firt procedure grew A. xylooxidan and P. acne. Culture from the vitrectomy grew only A. xylooxidan. At the final follow-up viit 6 month after the initial procedure. The eye wa without inflammation with bet-corrected viual acuity of 20/40. CONCLUION: Both A. xylooxidan and P. acne can caue chronic progreive endophthalmiti after cataract extraction often reitant to corrective antibiotic therapy. ucceful intervention may require complete vitrectomy with intraocular len and capule removal
