Pot-traumatic endophthalmiti involving Clotridium tetani and Bacillu pp

PURPOE: To report a cae of pot-traumatic infectiou endophthalmiti caued by Clotridium tetani and Bacillu pp. METHOD: Cae report. REULT: A 25-year-old man developed endophthalmiti after a traumatic corneocleral laceration of hi right eye by a concrete reinforcement bar. He underwent par plana lenectomy and vitrectomy with apiration of vitreou fluid and a conjunctival wab for culture. Culture from the conjunctival wab were negative for organim. Culture of the vitreou apirate were poitive for Bacillu pecie and C. tetani. He had received a tetanu toxoid booter at the emergency department. By the time the culture reult became available, he had developed evere eye pain aociated with marked orbital congetion, increaed welling and erythema of the lid, marked injection and chemoi of the conjunctiva, and ubequently underwent eviceration. The inflammation reolved after eviceration of the right eye, and he wa dicharged to home on doxycycline 100 mg orally two time daily for 10 day. CONCLUION: We are unaware of previou report of endophthalmiti involving C tetani and could find none in a computerized MEDLINE earch. Patient with penetrating eye injury hould be aeed for tetanu immunization tatu, and early intervention with tetanu toxoid booter and/or tetanu immune globulin hould be conidered if culture are poitive.
