Total en bloc exciion: a modified vitrectomy technique for proliferative diabetic retinopathy

PURPOE: To report a modified vitrectomy technique for proliferative diabetic retinopathy. DEIGN: Interventional cae erie. METHOD: In the modified vitrectomy technique for proliferative diabetic retinopathy, core vitrectomy i performed, followed by eparation of the glial ring from the optic dik uing a hook or forcep. The poterior hyaloid membrane, including proliferative tiue, i lifted and peeled from the poterior to the peripheral retina without creating a window in the poterior hyaloid membrane. At any trong vitreoretinal adheion ite, the poterior hyaloid i partially removed, followed by membrane delamination. REULT: In the 18 eye that underwent the modified technique, iatrogenic retinal break occurred le often compared with the 48 eye that underwent the conventional technique. The difference wa not tatitically ignificant (P = .14), but the urgical time wa ignificantly reduced (P = .0003). The glial ring and proliferative tiue ariing from the optic dik can be readily eparated, and other adheion ite can be atifactorily peeled after eparating the poterior hyaloid membrane around the optic dik. CONCLUION: Thi modified vitrectomy technique for proliferative diabetic retinopathy may be an alternative urgical technique to the conventional technique.
