Abstract
Purpoe
To propectively tudy the effect of the ue of Intac microthin precription inert for the management of keratoconu.
Deign
Propective nonrandomized clinical trial.
Method
Thirty-three eye of 26 keratoconu patient (17 male and 9 female) age 21 to 51 year (mean age, 32 ± 9.7 year) were included in the current tudy. All patient had clear central cornea and contact len intolerance. Patient were excluded if any of the following criteria applied after the preoperative examination: previou intraocular or corneal urgery; hitory of herpe keratiti; diagnoed autoimmune dieae; and ytemic connective tiue dieae. Two Intac egment of 0.45-mm thickne were inerted in the cornea of each eye, aiming at embracing the keratoconu area to try to achieve maximal flattening. Preoperative examination included uncorrected viual acuity (UCVA), bet-corrected viual acuity (BCVA), manifet refraction, keratometric data, and corneal topography.
Reult
Intac were uccefully implanted in all eye. In one eye Intac were removed after 3 month becaue of their improper (uperficial) placement. The follow-up ranged from 1 to 24 month (mean: 11.3 month). The mean UCVA ignificantly improved from 0.13 ± 0.14 (range, counting finger [CF]–0.5) to 0.39 ± 0.27 (range, CF-1.0) (P < .01). Of 33 eye, 2 eye lot 1 line of UCVA, and 3 eye maintained the preoperative UCVA, wherea the ret (28 eye) experienced a 1- to 10-line gain. The mean BCVA alo improved from 0.47 ± 0.31 (range, CF-1.0) to 0.64 ± 0.26 (range, 0.1-1.0) (P < .01). Of 33 eye, 4 eye experienced 1- to 2-line lo of BCVA, 4 eye maintained the preoperative BCVA, wherea the ret (25 eye), experienced a 1- to 6-line gain. Of 3 patient (3 eye) with unatifactory reult, 1 patient improved with one egment removal and in 2 patient the egment were permanently removed. One of thee eye underwent ucceful PKP.
Concluion
With mean follow-up of 11.3 month, intracorneal ring egment implantation improved UCVA and BCVA in the majority of the keratoconu patient. Even though the reult are encouraging, concern till exit regarding the predictability a well a the long-term effect of uch an approach for the management of keratoconu.
