  • Abstract
    Purpoe We previouly demontrated that elective uture removal reduce keratoplaty atigmatim; however, a myopic hift wa induced with the increaing number of interrupted uture removed. Thi tudy i an attempt to determine the effect of a modified urgical technique on potkeratoplaty myopia, atigmatim, and aniometropia. Deign A cohort tudy compared with hitorical control. Method Optical penetrating keratoplatie were performed on 92 eye of 84 patient. The tudy group conited of 92 conecutive penetrating keratoplatie performed uing 12 interrupted 10-0 nylon uture and a tight 12-bite continuou uture and an average K reading of 46.00 diopter for eye undergoing combined and intraocular len exchange procedure. All patient had refraction, keratometry, and videokeratocopy potoperatively tarting at 6 week and at the completion of elective uture removal. Reult Before uture removal, the average pherical equivalent wa −0.160 ± 3.59 diopter; it wa −1.58 ± 3.66 diopter at the completion of uture removal at 1 year and −1.44 ± 3.72 at the lat follow-up viit, averaging 20.7 month. Final refractive, keratometric, and videokeratocopy atigmatim wa 2.81 ± 1.82, 4.19 ± 2.94, and 3.58 ± 2.03 diopter, repectively. Aniometropia, uing the pherical equivalent of the operated and fellow eye, wa 2.49 ± 2.25 diopter at completion of the tudy. A bet-corrected viual acuity of 20/50 or better wa achieved in 59% of patient. Concluion Low myopic pherical equivalent and aniometropia were achieved uing a K reading of 46 diopter for calculation of intraocular len power. The ue of tighter continuou uture and elective removal of fewer interrupted uture only erved to increae final atigmatim, with no ignificant effect on the final pherical equivalent.
