Diplacement of Rectu Mucle Pulley by Torional Mucle urgery for Treatment of Full Macular Tranlocation-induced Incyclotropia

Purpoe To elucidate the coronal plane location of extraocular mucle (EOM) pulley following torional mucle urgery. Deign Cae report. Method A 76-year-old man underwent advancement of the anterior part of the inferior oblique mucle to treat full macular tranlocation–induced incyclotropia. Potoperatively, magnetic reonance imaging wa ued to obtain contiguou, 2-mm-thick coronal orbital image. On each MRI image, the cro-ectional area and center of the EOM wa computed, and all rectu EOM poition were tranlated to the coordinate origin at the area centroid of the globe at the level of pulley. Reult The uperior rectu pulley wa diplaced temporally, and the lateral rectu pulley wa diplaced inferiorly more than 2 D from normal ubject. Coronal plane location of EOM pulley of the ipilateral eye howed extorion compared with that of the contraleional eye. Concluion Torional mucle urgery caue an extorional hift of the uperior and lateral rectu pulley.
