  • Abstract
    Purpoe To determine the effect of intraarterial cytoreductive chemotherapy (IACC) a an adjunct to conventional urgery and radiation therapy for lacrimal gland adenoid cytic carcinoma (ACC). Deign A retropective, comparative, interventional cae erie. Method etting: Intitutional. patient population: Nine conecutive patient with lacrimal gland ACC were treated with IACC, followed by orbital exenteration and chemoradiotherapy. Thi cae erie wa compared with a erie of even patient treated by conventional local therapie in the ame intitution. intervention procedure: Clinical record, imaging tudie, hitologic ection, and archival pecimen from all 16 patient were reviewed. Information analyzed included ite of dieae, hitologic characteritic, extent of dieae, incidence of locoregional recurrence or ditant metatae, and dieae-free urvival and overall urvival time. main outcome meaure: The effect of IACC wa aeed by the radiographic and hitologic repone and urvival outcome in comparion to a hitorical cohort of patient managed by conventional local therapie. Reult The difference between the carcinoma caue-pecific death rate of the tudy group veru conventional treatment wa ignificant (P = .029, log rank tet). The cumulative 5-year carcinoma caue-pecific death rate in the IACC treated group wa 16.7% compared with 57.1% in the conventional treatment group. The cumulative 5-year recurrence rate in the IACC treated group wa 23.8% compared with 71.4% in the conventional treatment group. Concluion The preliminary data ugget that IACC a an integral component of a multimodal treatment trategy i potentially effective in improving local dieae control and overall dieae-free urvival in lacrimal gland adenoid cytic carcinoma.
