گزارش يك مورد روان‌پريشي و بازگيري ترامادول

چكيده ترامادول هيدروكلرايد يك ضد درد اپيوييدي فعال كننده مركزي است كه براي كنترل درد متوسط تا شديد به كار مي?رود و نسبت به اپيوييدها مزاياي بهتري و عوارض كمتري دارد. اين مقاله به معرفي بيماري با وابستگي به ترامادول مي‌پردازد كه به دنبال قطع ناگهاني آن دچار روان‌پريشي شده بود. با توجه به استفاده بي‌رويه ترامادول در تسكين دردهاي مزمن موارد روز افزون از سو مصرف و وابستگي به آن مشاهده شده است. به دليل تاثير ترامادول بر گيرنده‌هاي كاتكولامين و سروتونين علاوه بر گيرنده‌هاي اپيوييدي برخي از علايم بازگيري آن مربوط به اپيوييدها نيست و در كنار علايم طبيعي بازگيري مواردي از علايم بازگيري غير طبيعي نيز گزارش شده است. از جمله اين علايم، روان‌پريشي مي‌باشد كه بعد از چند روز با فروكش علايم بازگيري، روان‌پريشي نيز بهبود مي?يابد؛ بنابراين به نظر مي‌رسد در اين موارد به جاي تشخيص اختلالات روان‌پريشي متعاقب بازگيري مواد، تشخيص ترك مواد (ترامادول) بيشتر مطرح است و بايد درمان را طبق اين تشخيص انجام داد.

Abstract Tramadol is a centrally acting opioid analgesic used to treat moderate to sever pain. It has more advantage and less opioid adverse effects than conventional opioid analgesia. This article reports a patient with tramadol dependency that had psychosis after tramadol withdrawal. By the increase of tramadol usage for relief of chronic pain, tramadol abuse and dependency is increased. Some of tramadol withdrawal symptoms are not related to opioid, for example when the effectiveness is not only on opioid receptors, but on catecholamine and serotonin receptors. So, together with typical symptoms of withdrawal, atypical symptomes had been reported. Psychosis is one of tramadol atypical withdrawal symptoms which subsided a few days after suppression of withdrawal symptoms. In such cases, the diagnosis is substance withdrawal instead of psychotic disorder due to substance withdrawal and treatment is based on this diagnosis.
