    Evaluation of Inhalation Technique of Metered Dose Inhaler(MDI) and Dry Powder Inhaler-TurbuhalerR (DPI) amongPediatric Patients with Asthma

    Background: A large number of asthmatic patients particularly children use their inhaler devices inappropriately. Therefore, recent asthma management guidelines recommend demonstrating the technique of inhaler's administration with concomitant correction upon each visit to every asthmatic patient. Objective: To evaluate the administration technique of Metered Dose Inhaler (MDI) and Dry Powder Inhaler (DPI) in asthmatic children. Method: The sample of this descriptive study included chronic asthmatic children who were on regular follow-up at pediatric asthma clinic in King Hussein Medical Center (KHMC). Children and/or their parents had a coaching demonstration of either the MDI technique or the DPI (TurbuhalerR) technique by the researcher. Then they were asked to demonstrate their inhalation technique (MDI or DPI), during which they were assessed using the standardized checklist of either of the techniques. Results: Fifty-six children were enrolled in the study, their age ranged between 11 and 14 years (mean age 12.5years). The sample included 30 males (55.6%). Two children (3.7%) were excluded since they could not complete the study. The most common mistake among MDI user was not holding breath for 10 seconds (60%). The other common mistake was absence of coordination of actuation and inhalation (46.6%). Not exhaling slowly prior to inhalation among DPI users was the most commonly made mistake (25%). This was followed equally by both improper positioning and twisting of grip or forceful and deep inhalation (8.3%). The median time of technique teaching was 5.7minutes for the MDI and 3.6 minutes for the DPI. More than two-third of the patients prefer DPI over MDI technique. Conclusion: To achieve adequate patient treatment outcome, it is better to use the dry powder inhaler (TurbuhalerR) since it is an easier technique with less teaching time. Regardless of the medication delivery system, a comprehensive inhalation instruction supported by health care providers (physician, pharmacist and nurse) is needed and inhalation technique must be checked repeatedly at follow-up visits to assure reliable inhalation technique in asthmatic children.
