Background & Objective: To study the prevalence of the diabetic hand in a group of type 2 diabetics, comparing to age- sex matched non-diabetics control group, and to investigate clinical features among diabetics with or without the diabetic hand.
Methods & Materials: A case-control descriptive study was carried out.
A group of 186 type 2 diabetics and a control group of 200 age – sex matched non-diabetics were included.
All candidates were either outpatients or inpatients who attended Almousat and Alassad Damascus university hospitals during one year period (November 2009 – November 2010).
Results: The average age of type 2 diabetic patients and non-diabetic controls was 55.02 ± 9.52 year and 56.99 ± 10.60 year respectively.
The duration of type 2 diabetes was one to 30 years (8.14 ± 6.29 year).
Hemoglobin A1C average was 7.55 ± 1.25 %.
When the prevalence of the diabetic hand was compared between type 2 diabetics and non-diabetics controls it was three folds and a half higher in the first group (26.34% vs 7.5%, P< 0.0001).
While limited joint mobility was the first most frequent among type 2 diabetics and it was 10 folds higher among them comparing to non-diabetics control (11.82% vs 1%, P<0.0001), flexor tenosynovitis was the second in frequency in type 2 diabetes and it was significantly higher (10.21%) compared to non-diabetics (4%) (P<0.05).
Dupuytren's contracture was rare (1.07% in diabetics vs non in the control).
The prevalence of carpal tunnel syndrome was nearly the same in diabetics and non-diabetics (3.22% vs 2.5%).
The diabetic hand was associated with both duration and poor control of type 2 diabetes (P<0.05), and it was also associated with diabetic neuropathy and nephropathy (P<0.003 and P< 0.0002 respectively).
However, it was not associated with diabetic cardiovascular complications and retinopathy.
Conclusion: The prevalence of diabetic hand was higher in type 2 diabetics compared to non-diabetics control, and it was associated with diabetes poor control, duration, nephropathy, and neuropathy.
Key words: Diabetic hand, diabetes mellitus type 2, limited joint mobility, flexor tenosynovitis, trigger finger, Dupuytren’s disease, and carpal tunnel syndrome
