    Insulin like growth factors- 1 (IGF- 1) is essential for longitudinal bone growth, as it stimulates proliferation and differentiation of chondrocytes in the epiphyseal plate. The aim of this study is to measure the serum IGF-1 level and predict as reliable marker in postmenopausal osteoporotic women. Serum IGF-1 level of 50 postmenopausal osteoporotic women (≥ 50 years ) with bone mineral density (BMD) (T score ≤ - 2.5) and that of 60 postmenopausal non osteoporotic women (≥ 50 years) with BMD (T score >; -1) were measured by ELISA method. Serum IGF-1 level of postmenopausal osteoporotic women (211.24 ± 36.30 ng/ml) was significantly lower than that of non osteoporotic women (- 365.15 ± 23.55 ng/ml) at p<;0.05*.BMD (Tscore) of postmenopausal osteoporotic women (- 3.18 ± 0.61) was also significantly lower than that of non osteoporotic women (- 0.56 ± 0.92) at p<; 0.05*. Serum IGF-1 level was correlated with BMD in postmenopausal osteoporotic women (r = 0.470, p = 0.001) as well as in postmenopausal non osteoporotic women (r = 0.346, p = 0.006) respectively. A decline of IGF-1 is one of the important mechanisms leading to the development of osteoporosis. Attempt should be made to identify a marker that not only changes earlier than BMD but also has some capacity to differentiate osteoporotic women with low bone mass from normal. IGF1 serves as a candidate.
