Survey on the Diagnostic Value of a Simple Scoring System in the Diagnosis of Acute Appendicitis

This study is aimed at evaluating the accuracy of a simple scoring system used for the diagnosis of acute appendicitis . 65 patients (above 20 years of age) with right lower quadrant pain requiring operation in Al-Zahra Medical center , of Esfahan Medical University , were selected . Operation was done after taking their history , physical examination and WBC count analysis . Required data included age , sex , duration of sympoms , genito-urinary symptoms , muscle spasm and rectal mass . Appendicitis was added to the checklist only after the pathology report . The results indicated that 60% of the patients were male ; mean age was 24.1 + 1.2 _ Also , in 93% , duration of symptoms was less than 1.5 days . Genito-urinary symptoms were negative in 98.4% . Muscle spasm was observed in 60% . WBC counts were less than 13000/cmm in 60% . The sensitivity of scoring symtem was 98% , its specificity 57.1% , positive predictive value 89% and negative predictive value 88% _ The findings of this study suggest that scoring system , useful for the diagnosis of acute appendicitis , can be used for decreasing the number of false negatives.
