Abstract :
The progress of technology has increased the number and capacity of chemical industries in recent years and resulted in their complexity. More complexity led to occurrence of more major industrial accidents. Due to consequences of these accidents a large amount of costs is imposed toward industries and the environment will be suffered. One of the best methods used in process hazard analysis is fault tree analysis. The lack of complete data on failure rates, doubts in available data and vagueness may lead to uncertainty in results of traditional fault tree analysis. Fuzzy model has been introduced as an effective and popular tool for overcoming such uncertainty. Fuzzy logic was applied in modeling fault tree analysis. The approach showed that it could be accounted as a decision making and a benchmark for future failures. Applying this approach increased the safety of system and reduced the environmental impacts and maintenance costs. Rupture of C4-cut storage tank during operation was defined as a top event of the fault tree. The fault tree was found to consist of 14 basic events. Fuzzy probability of basic events was computed and the occurrence probability of top event was found to be 0.00371. The study of rupture of C4-cut storage tank during operation indicated that corrosion, flashing of liquid in tank and failure of pressure indicator played important roles in failure of the storage tank.
