Traitements préopératoires des cancers rectaux

Résumé rurgie seule nʹest plus adaptée au traitement des cancers rectaux classés T3–T4 résécables. La chimioradiothérapie préopératoire est devenue le nouveau traitement standard. Cette approche fait chuter le taux des récidives locales à 6–8 %. Néanmoins, cette approche ne se traduit pas en augmentation de la probabilité de survie globale. De nouvelles approches doivent donc être développées, de même la recherche doit se tourner vers la compréhension des mécanismes biologiques susceptibles dʹexpliquer la « résistance » aux traitements dʹinduction. y alone is no longer appropriate to the treatment of T3–T4 resecable rectal cancer. Preoperative chemoradiotherapy has recently been approved as the new standard treatment. This approach improves local control with local failure rate raranging now around 6–8%. However, it does not impact on overall survival. It becomes urgent to develop new concepts and a basic research in the understanding of the biological mechanisms that may explain the resistance of the micrometastatic process.
