Métastase intramammaire d’un cancer vulvaire : à propos d’un cas avec revue de la littérature

Résumé cer vulvaire représente moins de 1 % de l’ensemble des cancers et 3 à 5 % des cancers génitaux de la femme. Il survient le plus souvent chez la femme âgée dans un contexte de carence estrogénique, habituellement précédé par des lésions précancéreuses. C’est un cancer souvent plurifocal et son extension se fait vers les ganglions lymphatiques régionaux. Il est rarement métastatique à distance. Nous rapportons le cas d’une patiente âgée de 42 ans ayant été suivie pour un carcinome épidermoïde vulvaire d’emblée métastatique dans les ganglions inguinaux traité par vulvectomie totale et curage ganglionnaire bilatéral suivis d’une radiothérapie externe du périnée et des ganglions inguinaux. Trois mois après la fin de son traitement, la patiente s’est présentée avec un nodule du sein gauche et un paquet ganglionnaire cervical droit. La biopsie des ganglions cervicaux droits a fait la preuve d’un carcinome épidermoïde. Le même type histologique a été retrouvé au niveau de l’examen extemporané puis sur la pièce de mastectomie de type Patey. L’origine du carcinome intramammaire a été attribuée à la vulve. Le traitement a consisté en une chimiothérapie type cyclophosphamide, adriblastine, cisplatine (CAP). Après trois cures de chimiothérapie, une réponse de plus de 75 % a été obtenue au niveau des ganglions cervicaux. La patiente est décédée dans un tableau de métastases cérébrales à 11 mois. Nous avons réalisé, à travers cette observation, une revue de la littérature concernant les particularités des formes métastatiques du carcinome épidermoïde vulvaire. east metastases resulting of vulvar carcinoma are very rare, and represent exceptionally the first manifestation of the disease. We report the case of a 42 year-old patient who underwent a treatment because of vulvar epidermoid carcinoma, right away metastatic at the level of the inguinal ganglia. The treatment consisted in a total vulvectomy with bilateral ganglial curretage, followed by external radiotherapy about the perineum and the inguinal ganglia. Three months after the end of her treatment, the patient presented with a nodula on the left outer breast with features of malignancy noticed by clinic and mammographic examination. The histologic study of the mammary biopsy showed epidermoid carcinoma of likely metastatic origin. A left Patey has been realized and confirmed the metastatic localization of epidermoid carcinoma with axillary ganglial metastasis (2N ± 7). Besides, this patient presented a right cervical ganglial parcel that the biopsy showed a metastatic localization of a vulvar carcinoma. A palliative chemotherapy type cyclophosphamid, adriblastin, cisplatine (CAP) has been admistrated during three cycles spaced out three weeks. The patient died 11 months after the supervene of the cerebral metastasis. We present this case because its rarety and to show the possibility of metastasis at the level of breast due to vulvar cancer. The clinicians must remember this possible tropism of the vulvar cancer for the breast, not only during the supervision and the complete examination as regards the disease spreading but also when the affection revealed unknown primary tumor. The diagnostic orientation is based on the mammography and the mammary biopsy. In this stage, the treatment is unfortunately palliative, the survival until a year is not more than 20%.
