Reducing the risk of failure by better training and education

Failures in engineering structures and components can arise for a number of reasons. Various factors associated with human issues are examined in this paper and some means of mitigating them are discussed. The importance of improvements in training and education in order to achieve a reduction in the risk of failure is emphasised.
