Experiences of Opium Dependents from Performance of Methadone Centers of Kerman, Iran

چكيده جهت رضايت سنجي و ارزيابي طرح درمان نگهدارنده متادون، درك چگونگي تجارب حين درمان افراد وابسته به مواد افيوني از عملكرد مراكز توزيع متادون اهميت داشته، باعث فهم بهتر و عميق‌تر كيفيت اين طرح درماني و تجديد نظر در استانداردهاي مراكز درمان متادون مي‌گردد. اين مطالعه با هدف توصيف ماهيت و ساختار تجربه از عملكرد مراكز درمان با متادون حين درمان انجام شد. اين مطالعه از نوع كيفي بود و به روش فنومنولوژي انجام گرفت. نمونه‌گيري مبتني بر هدف بود و از بين افراد وابسته به مواد افيوني مراجعه كننده به مراكز متادون شهر كرمان در سال 1386 انتخاب گرديد. نمونه‌گيري تا اشباع اطلاعات ادامه يافت و حجم نمونه به 32 نفر رسيد. براي تجزيه و تحليل اطلاعات، روش Colaizzi به كار رفت. حاصل اين پژوهش 27 كد بود كه در چهار دسته موضوعي اصلي تجربه از ساختار، پرسنل، مراجعين و قوانين مركز قرار گرفت. اين چهار دسته موضوعي ساختار اصلي تجربه از عملكرد مراكز توزيع متادون را نشان مي‌داد. نبودن مراكز درماني و يا مشكلات زياد در مراكز درماني در شهرهاي اطراف، لزوم راه‌اندازي و يا رفع مشكلات مراكز شهرستان‌ها را نشان مي‌دهد. تركيب مراجعين مراكز نيز در سير درمان نقش دارد. حضور منظم پزشك و ساير پرسنل و رسيدگي‌هاي بيشتر آن‌ها و ساعات بيشتر كاري مركز در رضايتمندي مراجعين و افزايش ماندگاري در درمان نيز نقش دارد. بحث و گفتگو با مراجعين در موارد قوانين مركز از جمله اجبار در مصرف دارو در مراكز و رفت و آمد روزانه به مركز جهت مصرف دارو و كوبيدن قرص‌ها لازم به نظر مي‌رسد. همچنين تدابير لازم جهت رفع مشكلات آزمايش ادرار ضروري مي‌باشد.

Abstract To assess patientsʹ satisfaction and to evaluate methadone therapy program, it is important to understand the experiences of opium dependents during the treatment period in methadone centers and determine the quality of this program and revise standards based on that. This study aimed to describe the nature and structure of patientsʹ experiences during treatment in methadone centers. This was a qualitative method using phenomenology. Sampling was purposive and the participants were selected from opium dependents referred to Kerman methadone centers during 2007. Sampling continued until data saturation and the sample size was 32. Colaizziʹs method was applied for data analysis. The findings of this study included 27 codes categorized in four main groups: experiences of structure, personnel, patients, and regulations. These four categories showed the main structure of experiences in methadone centers. Lack of treatment centers in near-by cities or the problems of those centers suggest that it is necessary to establish new centers or solve the problems of centers in near-by cities. The type of patients referring to the centers plays a role in treatment process. The regular presence of physicians and other personnel and their concerns and care for patients as well as longer working hours of the centers have roles in patientsʹ satisfaction and increase of consistency with treatment. Discussing the rules and regulations of the center with patients including the obligatory of daily reference to the center to obtain medicine and injections sound necessary. Also, it is necessary to find ways for solving problems of urine tests.
