Abstract :
Objectives: To evaluate if epidural administration of xylazine could decrease the effective dose of intravenous ketamine for maintenance of general anesthesia in laparoscopic procedures and assessment of related complications during anesthesia. Design: Experimental Study. Animals: 20 Female dogs with 21.0 ± 3.0 kg weight and 18.0 ± 1.2 months age. Material and methods: Animals randomly divided into two groups (n=10). All dogs premedicated with acepromazine. Anesthesia induced by combination of ketamine and diazepam. In one group xylazine injected epidurally then all of them spayed by same laparoscopic method and repeated doses of ketamine for maintenance of anesthesia. Cardiopulmonary parameters such as SpO2 and ETCO2 during operation were recorded. Results: There were no significant differences in weight and age between two groups. Mean of ketamine administration in group 1 was 640 mg and in group 2 (have epidural administration of xylazine) was 270 mg Conclusions and Clinical Relevance: Data analysis showed that epidural administration of xylazine, decreases the amount of ketamine needed to maintain anesthesia (P<0.05) with no significant difference in cardio-pulmonary parameters during laparoscopic operation. Epidural injection of Xylazine could be used as a concurrent medication in general anesthesia without complication. Analgesic effects of xylazine decreased the use of ketamine (P<0.05) by repeated dose based on intra operative pain reflex during anesthesia.

ﻫﺪف- اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﺑﻪ ﻣﻨﻈﻮر ﺑﺮرﺳﻲ اﺛﺮ ﺗﺠﻮﻳﺰ اﭘﻲ دورال زاﻳﻼزﻳﻦ ﺑﺮ ﻣﻴﺰان ﻛﺘـﺎﻣﻴﻦ ورﻳـﺪي ﻣـﻮرد ﻧﻴـﺎز ﺑـﺮاي ﺑﻴﻬﻮﺷـﻲ ﻋﻤـﻮﻣﻲ در ﺟﺮاﺣﻲ ﻫﺎي ﻻﭘﺎروﺳﻜﻮﭘﻴﻚ و ارزﻳﺎﺑﻲ ﭘﻴﭽﻴﺪﮔﻲ ﻫﺎي اﺣﺘﻤﺎﻟﻲ ﺣﻴﻦ ﻋﻤﻞ، ﺑﻪ اﻧﺠﺎم رﺳﻴﺪ. ﻧﻮع ﻣﻄﺎﻟﻌﻪ- ﻣﻄﺎﻟﻌﻪ ﺗﺠﺮﺑﻲ. ﺣﻴﻮان ﻣﻮرد ﻣﻄﺎﻟﻌﻪ- 20 ﺳﮓ ﻣﺎده ﺑﺎ وزن21±3 ﻛﻴﻠﻮﮔﺮم و ﺳﻦ 18±1/2 ﻣﺎه. ﻣﻮاد و روش ﻛﺎر- ﺣﻴﻮاﻧﺎت ﺑﻄﻮر ﺗﺼﺎدﻓﻲ ﺑﻪ دو ﮔﺮوه 10 ﺗﺎﻳﻲ ﺗﻘﺴﻴﻢ ﺷﺪﻧﺪ. ﺑﻪ ﻋﻨﻮان داروي ﭘﻴﺶ ﺑﻴﻬﻮﺷﻲ اﺳﭙﺮوﻣﺎزﻳﻦ ﺑﻪ ﺣﻴﻮﻧـﺎ ت ﺗﺠﻮﻳﺰ ﺷﺪ. ﺳﭙﺲ اﻟﻘﺎ ﺑﻴﻬﻮﺷﻲ ﺑﺎ ﺗﺠﻮﻳﺰ ﻛﺘﺎﻣﻴﻦ ﺑﻪ ﻫﻤﺮاه دﻳﺎزﭘﺎم ﺑﻪ اﻧﺠﺎم رﺳﻴﺪ. در ﻳﻚ ﮔﺮوه زاﻳﻼزﻳﻦ ﺑﺼﻮرت اﭘﻴﺪورال ﺗﺠـﻮﻳﺰ ﺷـﺪ ﺳﭙﺲ در ﺗﻤﺎم ﺣﻴﻮاﻧﺎت ﻋﻤﻞ ﺑﺮداﺷﺖ رﺣﻢ و ﺗﺨﻤﺪان ﻫﺎ ﺑـﺎ ﻳـﻚ روش ﻻﭘﺎروﺳـﻜﻮﭘﻴﻚ ﻳﻜﺴـﺎن ﺻـﻮرت ﭘـﺬﻳﺮﻓﺖ ﺑـﺮاي ﻧﮕﻬﺪاﺷـﺘﻦ ﺑﻴﻬﻮﺷﻲ از ﺗﻜﺮار ﺗﺠﻮﻳﺰ ﻛﺘﺎﻣﻴﻦ اﺳﺘﻔﺎده ﺷﺪ. رﻓﻠﻜﺲ ﻫﺎ ي ﭘﻠﻜﻲ و ﻗﺮﻧﻴﻪ اي و ﭘﺎراﻣﺘﺮﻫﺎي ﻗﻠﺒﻲ-ﺗﻨﻔﺴﻲ ﻣﺎﻧﻨـﺪ اﺷـﺒﺎع ﻫﻤﻮﮔﻠـﻮﺑﻴﻦ از اﻛﺴﻴﮋن )SpO2( و ETCO2 در ﻃﻲ ﺟﺮاﺣﻲ ﺛﺒﺖ ﺷﺪﻧﺪ. ﻧﺘﺎﻳﺞ- ﺳﮓ ﻫﺎي ﻣﻮرد اﺳﺘﻔﺎده در اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﺗﻔﺎوت ﻣﻌﻨﻲ داري از ﻧﻈﺮ وزن وﺳﻦ ﻧﺪاﺷﺘﻨﺪ. ﻣﺘﻮﺳﻂ ﻣﺼﺮف ﻛﺘـﺎﻣ ﻴﻦ در ﺳـﮓ ﻫـﺎي ﮔﺮوه 1 در ﻣﺠﻤﻮع 640 ﻣﻴﻠﻲ ﮔﺮم ﺑﻮد و در ﺳﮓ ﻫﺎي ﮔﺮوه 2 ﮔﺮوﻫﻲ ﻛﻪ در آﻧﻬﺎ ﺗﺠﻮﻳﺰ اﭘﻲ دورال زاﻳﻼزﻳـﻦ اﻧﺠـﺎم ﻣـﻲ ﺷـﺪ 270 ﻣﻴﻠﻲ ﮔﺮم. ﻧﺘﻴﺠﻪ ﮔﻴﺮي- ﺗﺤﻠﻴﻞ داده ﻫﺎ ﻧﺸﺎن داد ﻛﻪ ﺗﺠﻮﻳﺰ زاﻳﻼزﻳﻦ ﻣﻴﺰان ﻛﺘﺎﻣﻴﻦ ﻣﻮرد ﻧﻴﺎز ﺑﺮاي ﻧﮕﻬﺪاري ﺑﻴﻬﻮﺷـﻲ را در ﺳـﮓ ﻫـﺎي ﻣـﻮرد ﻣﻄﺎﻟﻌﻪ ﻛﺎﻫﺶ ﻣﻲ دﻫﺪ، ﺑﺪون اﻳﻨﻜﻪ اﺛﺮ ﻣﻌﻨﻲ داري ﺑﺮ ﭘﺎراﻣﺘﺮﻫﺎي ﻗﻠﺒﻲ-ﺗﻨﻔﺴﻲ داﺷﺘﻪ ﺑﺎﺷﺪ. ﻣﻲ ﺗﻮان از ﺗﺠـﻮﻳﺰ ﻫﻤﺰﻣـﺎن اﭘـﻲ دورال زاﻳﻼزﻳﻦ ﺑﺎ ﺑﻴﻬﻮش ﻛﻨﻨﺪه ﻫﺎي ﻋﻤﻮﻣﻲ ﺑﺮاي ﻛﺎﻫﺶ ﻣﻴﺰان داروي ﺑﻴﻬﻮش ﻛﻨﻨﺪه ﻋﻤﻮﻣﻲ ﻣـﻮرد ﻧﻴـﺎز ﺑـﺪون اﻳﺠـﺎد ﭘﻴﭽﻴـﺪﮔﻲ ﺧﺎﺻـﻲ اﺳﺘﻔﺎده ﻛﺮد. اﺛﺮات ﺑﻲ درد ﻛﻨﻨﺪه زاﻳﻼزﻳﻦ ﻣﻲ ﺗﻮاﻧﺪ ﻣﻴﺰان ﻧﻴﺎز ﺑﻪ ﻛﺘﺎﻣﻴﻦ را در ﻃﻲ ﺑﻴﻬﻮﺷﻲ ﻋﻤﻮﻣﻲ ﻛﺎﻫﺶ دﻫﺪ.
