Abstract :
Objective: The aim of this study was to compare the use of Polypropylene mesh
(Prolene) and Sepramesh, a coated Polypropylene mesh with a protective layer of
Seprafilm on its visceral side, for the repair of abdominal wall defects in horses. We also
aimed to quantify the consequent visceral adhesion and tissue inflammation.
Study Design: Experimental study.
Animal Population: Ten horses.
Methods: The horses were divided into the control group, where a 4×8 cm defect was
created through the midline of the abdomen and repaired with polypropylene mesh, and
the experimental group, where the same defect was made and closed sepramesh. Both
meshes were placed intraperitoneally and sutured to the cut margins of peritoneum and
the opponeurosis of external abdominal oblique muscle contacting in viscera in a tensionfree
technique.
Results: The severity and extent of adhesions were significantly lower in the
experimental group (B) than the control group (A) (P<0.05). Horses that received a Polypropylene mesh experienced higher levels of inflammation, both on the day of
operation and at two weeks, but significant differences were not apparent after 4 weeks.
Conclusions: This study confirmed the advantages of Sepramesh over Polypropylene
mesh in the repair of abdominal wall defects in horses.
Clinical Relevance: There are many causes of abdominal wall defects in horses,
including congenital and traumatic. This experiment suggests that the use of Sepramesh
could strengthen the healing of abdominal wounds, prevent incisional hernias, and reduce
intraabdominal adhesions.

ﻫﺪف- ﻫﺪف از اﻧﺠﺎم اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﻣﻘﺎﻳﺴﻪ ﻛﺎرﺑﺮد ﺗﻮري ﺟﺮاﺣﻲ ﭘﻠﻲ ﭘﺮوﭘﻴﻠﻦ )ﭘﺮوﻟﻦ( و ﺳﭙﺮاﻣﺶ )ﺗﻮري ﭘﻠﻲ ﭘﺮوﭘﻴﻠﻦ ﭘﻮﺷﺶ ﻳﺎﻓﺘﻪ ﺑﺎ ﻏﺸﺎء ﻣﺤﺎﻓﻆ از ﺳﭙﺮاﻓﻴﻠﻢ در ﺳﻤﺖ اﺣﺸﺎﺋﻲ( ﺑﺮاي ﺗﺮﻣﻴﻢ ﻧﻘﻴﺼﻪ دﻳﻮاره ﺷﻜﻢ در اﺳﺐ ﺑﻮد. ﻫﻤﭽﻨﻴﻦ ﻛﻴﻔﻴﺖ ﭼﺴﺒﻨﺪﮔﻲ اﺣﺸﺎﻳﻲ و اﻟﺘﻬﺎب ﺑﺎﻓﺘﻲ در ﻣﺤﻞ ﻧﻘﻴﺼﻪ ﻣﻮرد ﺑﺮرﺳﻲ ﻗﺮار ﮔﺮﻓﺖ.
ﻃﺮح ﻣﻄﺎﻟﻌﻪ- ﻣﻄﺎﻟﻌﻪ ﺗﺠﺮﺑﻲ
ﺣﻴﻮاﻧﺎت- ده راس اﺳﺐ
روش ﺑﺮرﺳﻲ- اﺳﺐ ﻫﺎ ﺑﻪ دو ﮔﺮوه ﻛﻨﺘﺮل و آزﻣﻮن ﺗﻘﺴﻴﻢ ﺷﺪﻧﺪ. ﻧﻘﻴﺼﻪ اي ﺗﻤﺎم ﺿﺨﺎﻣﺖ ﺑﻪ اﺑﻌﺎد 4 × 8 ﺳﺎﻧﺘﻲ ﻣﺘﺮ در ﻣﺤﻞ ﺧﻂ ﻣﻴﺎﻧﻲ ﺷﻜﻢ اﻳﺠﺎد ﺷﺪ ﻛﻪ در ﮔﺮوه ﻛﻨﺘﺮل ﺗﻮﺳﻂ ﺗﻮري ﭘﻠﻲ ﭘﺮوﭘﻴﻠﻦ و در ﮔﺮوه آزﻣﻮن ﺗﻮﺳﻂ ﺗﻮري ﺳﭙﺮاﻣﺶ ﺗﺮﻣﻴﻢ ﺷﺪ. ﻫﺮ دو ﺗﻮري ﺟﺮاﺣﻲ ﻃﺒﻖ ﺷﻴﻮه ﻋﺎري از ﻛﺸﺶ و ﺑﻪ ﺻﻮرت داﺧﻞ ﺻﻔﺎﻗﻲ و ﺑﻪ ﻟﺒﻪ ﺑﺮﻳﺪه ﺻﻔﺎق و آﭘﻮﻧﻮروز ﻋﻀﻠﻪ ﻣﻮرب ﺧﺎرﺟﻲ ﺷﻜﻢ ﺑﻪ ﻃﻮري ﻛﻪ ﺑﺎ اﺣﺸﺎء در ﺗﻤﺎس ﺑﺎﺷﺪ ﻛﺎرﮔﺬاري ﺷﺪﻧﺪ.
ﻧﺘﺎﻳﺞ- ﺷﺪت و ﮔﺴﺘﺮه ﭼﺴﺒﻨﺪﮔﻲ ﺑﻪ ﻃﻮر ﻣﻌﻨﻲ داري در ﮔﺮوه آزﻣﻮن ﻛﻤﺘﺮ از ﮔﺮوه ﻛﻨﺘﺮل ﺑﻮد )P<0.05(. اﺳﺐ ﻫﺎﻳﻲ ﻛﻪ ﺗﻮﺳﻂ ﺗﻮري ﭘﻠﻲ ﭘﺮوﭘﻴﻠﻦ درﻣﺎن ﺷﺪه ﺑﻮدﻧﺪ ﺳﻄﺢ ﺑﺎﻻﺗﺮي از اﻟﺘﻬﺎب را در روز ﻋﻤﻞ و دو ﻫﻔﺘﻪ ﺑﻌﺪ از آن ﺗﺠﺮﺑﻪ ﻛﺮدﻧﺪ. اﻣﺎ اﻳﻦ ﺗﻔﺎوت ﺑﻌﺪ از 4 ﻫﻔﺘﻪ ﻣﻌﻨﻲ دار ﻧﺒﻮد.
ﻧﺘﻴﺠﻪﮔﻴﺮي و ﻛﺎرﺑﺮد ﺑﺎﻟﻴﻨﻲ- ﻣﻮارد ﺑﺴﻴﺎر زﻳﺎدي از ﻧﻘﻴﺼﻪ ﻫﺎي دﻳﻮاره ﺷﻜﻢ در اﺳﺐ ﻫﺎ ﻣﺸﺎﻫﺪ ﺷﺪه ﻛﻪ ﻣﻲ ﺗﻮاﻧﺪ ﻣﺎدر زادي و ﻳﺎ اﻛﺘﺴﺎﺑﻲ ﺑﺎﺷﻨﺪ. اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﻣﺰﻳﺖ و ﺑﺮﺗﺮي ﺗﻮري ﺳﭙﺮاﻣﺶ را ﺑﻪ ﭘﻠﻲ ﭘﺮوﭘﻴﻠﻦ در ﺗﺮﻣﻴﻢ ﻧﻘﺎﻳﺺ دﻳﻮاره ﺷﻜﻢ در اﺳﺐ اﺛﺒﺎت ﻛﺮد. اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﭘﻴﺸﻨﻬﺎد ﻣﻲ ﻛﻨﺪ ﻛﻪ اﺳﺘﻔﺎده از ﺳﭙﺮاﻣﺶ ﻣﻲ ﺗﻮاﻧﺪ اﻟﺘﻴﺎم دﻳﻮاره ﺷﻜﻢ را ﺗﻘﻮﻳﺖ ﻛﺮده و ﺟﻠﻮي ﻓﺘﻖ ﺑﺮﺷﻲ و ﭼﺴﺒﻨﺪﮔﻲ داﺧﻞ ﺷﻜﻤﻲ را ﺑﮕﻴﺮد.
