Abstract :
Objective- The aim of this study was to demonstrate the efficacy of MSCs transplantation in combination with low level laser
irradiation (LLLI) in repair of experimental acute spinal cord injury.
Design- Experimental study.
Animals- 28 adult male Wistar Rats.
Procedures- A ballon- compression technique was used to produce an injury at the T8-T9 level of spinal cord applying Fogarty
embolectomy catheter. In group-1, the autologous MSCs were transplanted to the spinal cord lesion and followed by treatment with
low level laser irradiation during 15 consecutive days in group-2. The injured rats in third group were treated by LLLI alone. The
functional recovery was assessed using the Basso-Beattie-Bresnahan (BBB) locomotion scoring within 5 weeks.
Results-In these three treatment groups, the scores were significantly higher than control group. The differences between group-2
and two other treatment groups were statistically significant during all five weeks after treatment. There were no significant
differences in BBB score between group-1(MSCs) and group-3(LLLI) at 3rd, 4th and 5th weeks of treatment. According to the
histopathological findings, the best response was observed in group-2 (MSCs+LLLI) that repair of injured parts of dorsal funiculi
and less cavitation were occurred by proliferation of mesenchymal stem cells and their differentiation to glial cells especially
oligodendrocytes resulting in axon regeneration and relatively spinal cord recovery.
Conclusion and Clinical Relevance- The findings of present study demonstrated that concurrent use of LLLI and local
transplantation of MSCs exhibited profound effects on axon regeneration and revealed remarkable functional improvement. These
results suggested that MSCs characteristics could be influenced by low level laser irradiation, so this treatment might be as a useful
procedure for neural regeneration, although further detailed investigations need to be carried out particularly in clinical cases.

ﻫﺪف- ﻫﺪف از اﯾﻦ ﻣﻄﺎﻟﻌﻪ ﺑﺮرﺳﯽ ﻋﻤﻠﮑﺮدي و ﻫﯿﺴﺘﻮﭘﺎﺗﻮﻟﻮژﯾﮏ اﺛﺮات ﺗﻮأم ﭘﯿﻮﻧﺪ ﺧﻮدي ﺳﻠﻮل ﻫﺎي ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﻣﻐﺰ اﺳﺘﺨﻮان و درﻣﺎن ﺑﺎ ﻟﯿﺰر ﮐﻢ ﺗﻮان در ﺗﺮﻣﯿﻢ ﺿﺎﯾﻌﺎت ﺣﺎد ﻧﺨﺎع در رت ﺑﻮد. ﻃﺮح- ﻣﻄﺎﻟﻌﻪ ﺗﺠﺮﺑﯽ
ﺣﯿﻮاﻧﺎت- ﺗﻌﺪاد 28 ﺳﺮ رت ﻧﺮﺑﺎﻟﻎ ﻧﮋاد وﯾﺴﺘﺎر.
روش ﮐﺎر: آﺳﯿﺐ ﺣﺎد ﻧﺨﺎﻋﯽ ﺑﺎ اﯾﺠﺎد ﻓﺸﺎر ﺑﺮروي ﻧﺨﺎع ﺑﻮﺳﯿﻠﻪ ﮐﺎﺗﺘﺮ ﺑﺎﻟﻮن دار ﻓﻮﮔﺎرﺗﯽ در ﻣﺤﻞ ﻣﻬﺮه ﺳﯿﻨﻪ اي 8 ﯾﺎ 9 اﻟﻘﺎ ﺷﺪه و ﭘﺲ از 7 روز درﮔﺮوه اول، در ﻣﺤﻞ ﺿﺎﯾﻌﻪ ﻧﺨﺎﻋﯽ ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺧﻮدي ﺗﺰرﯾﻖ ﮔﺮدﯾﺪ، در ﮔﺮوه دوم ﻋﻼوه ﺑﺮ ﺗﺰرﯾﻖ ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺧﻮدي، ﺗﺤﺖ درﻣﺎن ﺑﺎ ﻟﯿﺰر ﮐﻢ ﺗﻮان ﻫﻢ ﻗﺮار ﮔﺮﻓﺘﻨﺪ. درﮔﺮوه ﺳﻮم ﺗﻨﻬﺎ درﻣﺎن ﺑﺎ ﻟﯿﺰر ﮐﻢ ﺗﻮان را درﯾﺎﻓﺖ ﻧﻤﻮدﻧﺪ. در ﮔﺮوه ﭼﻬﺎرم ﻫﯿﭻ درﻣﺎﻧﯽ اﻧﺠﺎم ﻧﺸﺪ ﮔﺮوه ﮐﻨﺘﺮل(. ﺑﺮرﺳﯽ ﻋﻤﻠﮑﺮدي رت ﻫﺎ در اﯾﻦ ﮔﺮوﻫﻬﺎ، در ﻣﺪت 5 ﻫﻔﺘﻪ ﺗﺎ زﻣﺎن ﺑﺮرﺳﯽ ﻫﯿﺴﺘﻮﭘﺎﺗﻮﻟﻮژﯾﮏ ﺑﺎﻓﺖ ﻧﺨﺎع، اﻧﺠﺎم ﮔﺮدﯾﺪ. ﻧﺘﺎﯾﺞ- ﺗﻐﯿﯿﺮات آﻧﺎﻟﯿﺰ رﻓﺘﺎري در ﻫﻔﺘﻪ ﻫﺎي اول ﺗﺎ ﭘﻨﺠﻢ ﺑﯿﻦ ﭼﻬﺎر ﮔﺮوه ﻣﻌﻨﯽ دار ﺑﻮد)0.001˂P(. اﺧﺘﻼف آﻣﺎري ﻣﻌﻨﯽ داري ﺑﯿﻦ ﮔﺮو ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ و ﮔﺮوه ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺑﻌﻼوه ﻟﯿﺰر ﮐﻢ ﺗﻮان؛ ﺑﯿﻦ ﮔﺮوه ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺑﻌﻼوه ﻟﯿﺰر ﮐﻢ ﺗﻮان و ﮔﺮوه ﻟﯿﺰر ﮐﻢ ﺗﻮان ﺗﻨﻬﺎ؛ ﺑﯿﻦ ﮔﺮوه ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺑﻌﻼوه ﻟﯿﺰر ﮐﻢ ﺗﻮان و ﮔﺮوه ﮐﻨﺘﺮل؛ ﺑﯿﻦ ﮔﺮوه ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ و ﮔﺮوه ﮐﻨﺘﺮل؛ ﻫﻤﭽﻨﯿﻦ ﺑﯿﻦ ﮔﺮوه ﻟﯿﺰر ﮐﻢ ﺗﻮان ﺗﻨﻬﺎ و ﮔﺮوه ﮐﻨﺘﺮل، در ﻫﻔﺘﻪ ﻫﺎي اول ﺗﺎ ﭘﻨﺠﻢ؛ ﻣﺸﺎﻫﺪه ﮔﺮدﯾﺪ. اﺧﺘﻼف آﻣﺎري آﻧﺎﻟﯿﺰ رﻓﺘﺎري ﺑﯿﻦ ﮔﺮوه ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ و ﻟﯿﺰر ﮐﻢ ﺗﻮان ﺗﻨﻬﺎ، ﻓﻘﻂ در ﻫﻔﺘﻪ ﻫﺎي اول و دوم ﻣﻌﻨﯽ دار ﺑﻮد. ﻧﺘﺎﯾﺞ ﻫﯿﺴﺘﻮﭘﺎﺗﻮﻟﻮژﯾﮏ ﻧﺸﺎن داد ﺑﻬﺘﺮﯾﻦ ﻧﺘﺎﯾﺞ ﻣﺮﺑﻮط ﺑﻪ ﮔﺮوه ﺗﺤﺖ درﻣﺎن ﺑﺎ ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺑﻪ ﻫﻤﺮاه ﻟﯿﺰر ﮐﻢ ﺗﻮان ﺑﻮد؛ ﺑﻄﻮري ﮐﻪ ﻗﺴﻤﺖ آﺳﯿﺐ دﯾﺪه ﻧﺨﺎع ﺗﺎ ﺣﺪي ﺗﺮﻣﯿﻢ و ﺑﺎزﺳﺎزي ﺷﺪه
ﺑﻮد. در ﻣﺎده ﺳﻔﯿﺪ، اﻓﺰاﯾﺶ ﺳﻠﻮﻟﻬﺎي ﮔﻠﯿﺎل، ﺑﺎزﺳﺎزي ﺳﻠﻮﻟﻬﺎي ﻣﯿﻠﯿﻦ واﮐﺴﻮﻧﻬﺎ ﻣﺸﺎﻫﺪه ﺷﺪ. ﺗﺮﻣﯿﻢ ﻧﻮروﻧﻬﺎ در ﺷﺎخ ﭘﺸﺘﯽ ﻣﺸﺎﻫﺪه ﻧﺸﺪ و ﻫﻤﺎﻧﻨﺪ ﮔﺮوه ﮐﻨﺘﺮل ﻫﯿﭙﻮﺳﻠﻮﻻرﯾﺘﯽ ﺷﺪﯾﺪي در آﻧﻬﺎ دﯾﺪه ﺷﺪ. در ﮔﺮوه درﻣﺎن ﺑﺎ ﺳﻠﻮل ﺑﻨﯿﺎدي، ﻫﻨﻮز ﺑﻘﺎﯾﺎي ﺣﻀﻮر ﻣﺎﮐﻮﻓﺎژﻫﺎ ﯾﺎ ﺳﻠﻮﻟﻬﺎي ﻣﯿﮑﺮوﮔﻠﯿﺎل ﻫﻀﻢ ﮐﻨﻨﺪه ﻣﯿﻠﯿﻦ دﯾﺪه ﻣﯽ ﺷﺪ. در ﮔﺮوه ﻟﯿﺰر ﮐﻢ ﺗﻮان ﺗﻨﻬﺎ ﻫﻢ ﺗﺤﺮﯾﮏ ﺳﻠﻮﻟﻬﺎي ﭘﺸﺘﯿﺒﺎن ﺑﻪ ﺗﮑﺜﯿﺮ و ﻫﺎﯾﭙﺮﺗﺮوﻓﯽ و ﮐﻤﮏ ﺑﻪ
ﺑﺎزﺳﺎزي ﻧﺨﺎع ﻣﺸﻬﻮد ﺑﻮد، ﻫﺮﭼﻨﺪ ﺑﺎزﺳﺎزي ﮐﺎﻣﻞ ﻧﺒﻮد.
ﻧﺘﯿﺠﻪ ﮔﯿﺮي و ﮐﺎرﺑﺮد ﺑﺎﻟﯿﻨﯽ- ﺑﺮرﺳﯽ ﻋﻤﻠﮑﺮدي و ﻫﯿﺴﺘﻮﭘﺎﺗﻮﻟﻮژﯾﮏ ﺣﺎﮐﯽ از ﻧﺘﺎﯾﺞ ﺑﻬﺘﺮ در ﮔﺮوه درﻣﺎن ﺑﺎ ﺳﻠﻮل ﺑﻨﯿﺎدي ﻣﺰاﻧﺸﯿﻤﯽ ﺑﻪ ﻫﻤﺮاه
درﻣﺎن ﺑﺎ ﻟﯿﺰر ﮐﻢ ﺗﻮان ﻧﺴﺒﺖ ﺑﻪ ﺳﺎﯾﺮ ﮔﺮوﻫﻬﺎ ﺑﻮد، و ﻣﯽ ﺗﻮاﻧﺪ ﻧﻮﯾﺪﺑﺨﺶ ﯾﮏ درﻣﺎن ﮐﺎرآﻣﺪ ﺑﺮاي آﺳﯿﺒﻬﺎي ﻧﺨﺎﻋﯽ ﺑﺎﺷﺪ؛ اﮔﺮﭼﻪ ﻣﻄﺎﻟﻌﺎت ﺑﯿﺸﺘﺮ ﮐﺎﻣﻠﺘﺮي در ﻣﻘﺎﯾﺴﻪ ﺑﺎ ﺳﺎﯾﺮ روش ﻫﺎي درﻣﺎﻧﯽ ﻣﻮﺟﻮد ﺿﺮوري ﺑﻪ ﻧﻈﺮ ﻣﯽ رﺳﺪ.
