Abstract :
Objective- This study aims at validating the use of serum amyloid A (SAA) as biomarkers of surgical
stress using Quantitative ELISA in the Sahel goats. The experiment evaluated SAA profiles of Sahel
goats post Diazepam-Lidocaine (DLC) and Diazepam-Bupivacaine (DBC) local anaesthesia. Expressions
of SAA in the Sahel goat post rumen skin clamp fixation techniques of rumenotomy with Polyglycolic
acid sutures (PGA) and chromic catgut sutures were also expounded.
Design- Experimental Study.
Animals- Fifteen Sahel goats were randomly allocated into three groups A, B and C for quantitative
ELISA evaluation of Serum Amyloid A (SAA) profiles post local anaesthesia and laparo-rumenotomy, as
biomarkers of surgical stress.
Procedures- Diazepam at 0.2mg/kg was administered intravenously to sedate goats in groups A and B
with subsequent lidocaine HCl and bupivacaine inverted-L block anaesthesia respectively. Chromic
catgut (CCG) and Polyglycolic Acid (PGA) sutures were used for rumen and abdominal muscle closures
for groups A and B respectively while nylon for skin apposition. Blood samples were collected via
jugular venipuncture to establish baseline data after local anaesthesia (PAI) and at 0, 5, 8, 24, 48 and 72
hours after surgery.
Results- Group A peak SAA values was 9.29±0.43 μg/mL at 8hrs post-surgery while goats in group B
had a peak of 10.94±1.22 μg/mL SAA at 24hrs post-surgery. SAA responses of group A indicates stress
at 0hr steadily throughout the first 72 hours but peaked at 8hrs post-surgery. Similarly, group B showed
significant stress at 8 hours onwards with peak values at 24 and 48 hours post-surgery.
Conclusion and Clinical Relevance- SAA as an acute phase protein could be used to determine early
complications following laparo-rumenotomy in the goat and offers good prognosis especially when
several APP variables are combined in an index.

ﻫﺪف- ﺑﺮرﺳﻲ اﺳﺘﻔﺎده از آﻣﻴﻠﻮﺋﻴﺪ A ﺳﺮﻣﻲ )SAA( ﺑﻪﻋﻨﻮان ﺑﻴﻮﻣﺎرﻛﺮﻫﺎي اﺳﺘﺮس ﺟﺮاﺣﻲ ﺑﺎ اﺳﺘﻔﺎده از اﻻﻳﺰا ﻛﻤﻲ در ﺑﺰﻫﺎي ﻧﮋاد ﺳﺎﺣﻞ. در اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﭘﺮوﻓﺎﻳﻞ SAA ﺑﻌﺪ از ﺑﻲﺣﺴﻲ ﻣﻮﺿﻌﻲ ﺑﺎ دﻳﺎزﭘﺎم- ﻟﻴﺪوﻛﺎﺋﻴﻦ و دﻳﺎزﭘﺎم ﺑﻮﭘﻴﻮاﻛﺎﺋﻴﻦ ارزﻳﺎﺑﻲ ﺷﺪ. ﺑﻴﺎن SAA در ﺑﺰ ﺳﺎﺣﻞ ﺑﺪﻧﺒﺎل ﺗﻜﻨﻴﻚ ﻓﻴﻜﺴﺎﺳﻴﻮن ﭘﻮﺳﺖ ﺷﻜﻤﺒﻪ ﺑﺎ ﮔﻴﺮه در روﻣﻨﻮﺗﻮﻣﻲ ﺑﺎ ﺑﺨﻴﻪ ﭘﻠﻲﮔﻠﻴﻜﻮﻟﻴﻚ اﺳﻴﺪ و ﻛﺎتﮔﻮت ﻛﺮوﻣﻴﻚ ﻫﻤﭽﻨﻴﻦ ﺗﻮﺿﻴﺢ داده ﺷﺪه اﺳﺖ.
ﻃﺮح ﻣﻄﺎﻟﻌﻪ ﺗﺠﺮﺑﻲ.
ﺣﻴﻮاﻧﺎت- 15 ﺑﺰ ﻧﮋاد ﺳﺎﺣﻞ ﺑﻪﺻﻮرت اﺗﻔﺎﻗﻲ در ﺳﻪ ﮔﺮوه ﺑﺮاي ارزﻳﺎﺑﻲ اﻻﻳﺰا ﻛﻤﻲ ﭘﺮوﻓﺎﻳﻞ SAA ﺑﻪﻋﻨﻮان ﺑﻴﻮﻣﺎرﻛﺮﻫﺎي اﺳﺘﺮس ﺟﺮاﺣﻲ ﺑﻌﺪ از ﺑﻲﺣﺴﻲ ﻣﻮﺿﻌﻲ و ﻻﭘﺎرو-روﻣﻴﻨﻮﺗﻮﻣﻲ ﻗﺮار داده ﺷﺪﻧﺪ.
روش ﻛﺎر- دﻳﺎزﭘﺎم ﺑﺎ دز 0/2 ﻣﻴﻠﻲﮔﺮم ﺑﻪازاي ﻛﻴﻠﻮﮔﺮم ﺑﺼﻮرت داﺧﻞ رﮔﻲ ﺑﺮاي آرام ﺑﺨﺸﻲ ﺑﺰﻫﺎ در ﮔﺮوهﻫﺎي A و B ﺗﺠﻮﻳﺰ ﺷﺪ و ﻣﺘﻌﺎﻗﺒﺎ ﺑﻪ ﺗﺮﺗﻴﺐ ﺑﻲﺣﺴﻲ L ﻣﻌﻜﻮس ﺑﺎ اﺳﺘﻔﺎده از ﻟﻴﺪوﻛﺎﺋﻴﻦ HCL و ﺑﻮﭘﻴﻮاﻛﺎﺋﻴﻦ ﺑﺮاي ﮔﺮوﻫﺎي A و B اﺳﺘﻔﺎده ﺷﺪ. ﺑﺨﻴﻪﻫﺎ ﺑﺎ اﺳﺘﻔﺎده از ﻛﺎت-ﮔﻮت ﻛﺮوﻣﻴﻚ و ﭘﻠﻲﮔﻠﻴﻜﻮﻟﻴﻚ اﺳﻴﺪ ﺑﺮاي ﺑﺴﺘﻦ ﺷﻜﻤﺒﻪ و ﻋﻀﻼت ﺷﻜﻤﻲ ﺑﺮاي ﮔﺮوهﻫﺎي A و B ﺑﻪ ﺗﺮﺗﻴﺐ ﻣﻮرد اﺳﺘﻔﺎده ﻗﺮار ﮔﺮﻓﺖ. ﻧﺦ ﻧﺎﻳﻠﻮن ﺑﺮاي ﺑﺨﻴﻪ ﭘﻮﺳﺖ اﺳﺘﻔﺎده ﺷﺪ. ﻧﻤﻮﻧﻪﻫﺎي ﺧﻮن از ﻃﺮﻳﻖ ورﻳﺪ وداج ﺑﻪ ﻣﻨﻈﻮر ﺗﻌﻴﻴﻦ داده ﭘﺎﻳﻪ ﺑﻌﺪ از ﺑﻲﺣﺴﻲ ﻣﻮﺿﻌﻲ و در زﻣﺎنﻫﺎي ﺻﻔﺮ، 5، 8، 24، 72 ,48 ﺳﺎﻋﺖ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﺟﻤﻊآوري ﺷﺪ.
ﻧﺘﺎﻳﺞ- در ﮔﺮوه A ﭘﻴﻚ SAA در 8 ﺳﺎﻋﺖ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﺑﺮاﺑﺮ ﺑﺎ 9/29±0/43 ﻣﻴﻜﺮوﮔﺮم ﺑﻪ ازاي ﻣﻴﻠﻲﺗﺮ و در ﮔﺮوه B ﭘﻴﻚ ﺑﺮاﺑﺮ ﺑﺎ 10/94±1/22 ﻣﻴﻜﺮوﮔﺮم ﺑﻪ ازاي ﻣﻴﻠﻲﺗﺮ در 24 ﺳﺎﻋﺖ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﺑﻮد. ﭘﺎﺳﺦ SAA در ﮔﺮوه A ﺑﻴﺎﻧﮕﺮ اﺳﺘﺮس در زﻣﺎن ﺻﻔﺮ ﺑﻄﻮر ﭘﻴﻮﺳﺘﻪ در 72 ﺳﺎﻋﺖ اول ﺑﻮد اﻣﺎ ﭘﻴﻚ ﭘﺎﺳﺦ در 8 ﺳﺎﻋﺖ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﮔﺰارش ﺷﺪ. ﺑﻄﻮر ﻣﺸﺎﺑﻪ، ﮔﺮوه B اﺳﺘﺮس ﻣﻌﻨﻲداري را در 8 ﺳﺎﻋﺖ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﺑﺎ ﺑﺎﻻﺗﺮﻳﻦ ﻣﻘﺎدﻳﺮ در 24 و 48 ﺳﺎﻋﺖ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﻧﺸﺎن داد.
ﻧﺘﻴﺠﻪﮔﻴﺮي و ﻛﺎرﺑﺮد ﺑﺎﻟﻴﻨﻲ- SAA ﻣﻲﺗﻮاﻧﺪ ﺑﻪﻋﻨﻮان ﻳﻚ ﭘﺮوﺗﺌﻴﻦ ﻓﺎز ﺣﺎد ﺑﺮاي ﺗﻌﻴﻴﻦ ﻋﻮاﻗﺐ زودﻫﻨﮕﺎم ﺑﻪدﻧﺒﺎل ﻻﭘﺎرو-روﻣﻴﻨﻮﺗﻮﻣﻲ در ﺑﺰ اﺳﺘﻔﺎده ﺷﻮد و ﭘﻴﺶآﻛﻬﻲ ﺧﻮﺑﻲ را ﻣﺨﺼﻮﺻﺎ ﻫﻨﮕﺎﻣﻴﻜﻪ ﭼﻨﺪﻳﻦ ﻣﺘﻐﻴﻴﺮ APP ﻳﻜﻲ ﻣﻲﺷﻮﻧﺪ ﭘﻴﺸﻨﻬﺎد ﻣﻲﻛﻨﺪ.
