Abstract :
Case Description- Three domestic short haired cats (2, 5 and 4 years of age) were presented to the Madras Veterinary College Teaching Hospital, with the history of large chronic, full thickness wounds on the right lateral cervical, left caudal forearm and right lateral thoracic region respectively. Etiology of the wounds was trauma in two cases and thermal burns in the third case. Clinical findings- Physical examination revealed yellowish colored wound with putrid odour and purulent discharge. The subcutaneous tissues were exposed; necrotic tissues and foreign debris were noticed. Treatment and Outcome- In all the three cases, mechanical debridement of the open wound was done by lavaging with sterile normal saline (500 ml) mixed with amikacin sulphate (250 mg) with a 20 ml syringe fixed with a sterile 22 gauge needle to produce a lavaging pressure of 22 psi. Autologous platelet rich plasma was prepared, activated with calcium gluconate(10%) and applied on days 0, 7, 14 respectively on the open wound impregnated with chlorhexidene gauze and bandaging done. Once a healthy granulation bed was formed, reconstructive surgery was planned and performed to close the defect. Clinical relevance- Application of platelet rich plasma remains a viable option for enhancing the granulation tissue formation and early wound bed preparation, before large wounds could be reconstructed with skin flaps. All the above three cases were treated with autologous platelet rich plasma for early granulation and recipient bed preparation followed by reconstructive surgery.

ﺗﻮﺻﻴﻒ ﺑﻴﻤﺎر- ﺳﻪ ﻗﻼده ﮔﺮﺑﻪ ﻧﮋاد ﻣﻮﻛﻮﺗﺎه اﻫﻠﻲ )2، 5 و 4 ﺳﺎﻟﻪ( ﺑﻪ ﺑﻴﻤﺎرﺳﺘﺎن آﻣﻮزﺷﻲ داﻧﺸﻜﺪه داﻣﭙﺰﺷﻜﻲ ﻣﺎدرس ﺑﺎ ﺗﺎرﻳﺨﭽﻪ زﺧﻢﻫﺎي ﺑﺰرگ ﻣﺰﻣﻦ ﻛﻪ ﺗﻤﺎم ﺿﺨﺎﻣﺖ ﭘﻮﺳﺖ را درﮔﻴﺮ ﻛﺮده ﺑﻮد آورده ﺷﺪﻧﺪ، زﺧﻢﻫﺎ ﺑﻪ ﺗﺮﺗﻴﺐ در ﺳﻤﺖ راﺳﺖ ﮔﺮدن، ﺳﻤﺖ ﺧﻠﻔﻲ و ﭼﭗ ﺑﺎزو و ﻧﺎﺣﻴﻪ ﺟﺎﻧﺒﻲ راﺳﺖ ﺗﻮراﻛﺲ ﻗﺮار داﺷﺘﻨﺪ. ﻋﻠﺖ زﺧﻢﻫﺎ در دو ﻣﻮرد ﺗﺮوﻣﺎ و در ﻳﻚ ﻣﻮرد ﺳﻮﺧﺘﮕﻲ ﺑﻮد. ﻋﻼﻳﻢ ﺑﺎﻟﻴﻨﻲ- ﻣﻌﺎﻳﻨﻪ ﻓﻴﺰﻳﻜﻲ زﺧﻢﻫﺎي زرد رﻧﮓ ﺑﺎ ﺑﻮي ﺗﻌﻔﻦ و ﺗﺮﺷﺤﺎت ﭼﺮﻛﻲ را ﻧﺸﺎن داد. ﺑﺎﻓﺖﻫﺎي زﻳﺮﺟﻠﺪ ﻗﺎﺑﻞ روﻳﺖ ﺑﻮدﻧﺪ، ﺑﺎﻓﺖ-ﻫﺎي ﻧﻜﺮوز ﺷﺪه و ذرات ﺧﺎرﺟﻲ ﺟﻠﺐ ﺗﻮﺟﻪ ﻣﻲﻛﺮد. درﻣﺎن و ﻧﺘﻴﺠﻪ- در ﻫﻤﻪ ﻣﻮارد، ﭘﺎﻛﺴﺎزي ﻓﻴﺰﻳﻜﻲ ﺑﺎ ﺷﺴﺘﺸﻮ ﺑﻮﺳﻴﻠﻪ ﻧﺮﻣﺎل ﺳﺎﻟﻴﻦ اﺳﺘﺮﻳﻞ )500 ﻣﻴﻠﻲﻟﻴﺘﺮ( ﻣﺨﻠﻮط ﺑﺎ آﻣﻴﻜﺎﺳﻴﻦ ﺳﻮﻟﻔﺎت )250 ﻣﻴﻠﻲﮔﺮم( ﺑﺎ ﻳﻚ ﺳﺮﻧﮓ 20 ﺳﻲﺳﻲ و ﺳﺮﺳﺮﻧﮓ ﺳﺎﻳﺰ 22 ﺑﻪ ﻣﻨﻈﻮر اﻳﺠﺎد ﻓﺸﺎر psi 22 ﻫﻨﮕﺎم ﺷﺴﺘﺸﻮ اﻧﺠﺎم ﺷﺪ. ﭘﻼﺳﻤﺎي ﻏﻨﻲ ﺷﺪه ﺑﺎ ﭘﻼﻛﺖ اﺗﻮﻟﻮﮔﻮس آﻣﺎده ﺷﺪ، ﺑﺎ ﮔﻠﻮﻛﻮﻧﺎت ﻛﻠﺴﻴﻢ ﻓﻌﺎل ﺷﺪ و در روزﻫﺎي ﺻﻔﺮ، 7 و 14 روي زﺧﻢﻫﺎي ﺑﺎز ﻗﺮار داده ﺷﺪ و ﺑﺎﻧﺪاژ اﻧﺠﺎم ﺷﺪ. ﻫﻨﮕﺎﻣﻴﻜﻪ ﻳﻚ ﺑﺴﺘﺮ اﻟﺘﻴﺎﻣﻲ ﺳﺎﻟﻢ ﺗﺸﻜﻴﻞ ﺷﺪ، ﺟﺮاﺣﻲ ﺗﺮﻣﻴﻤﻲ ﺑﻪ ﻣﻨﻈﻮر ﺑﺴﺘﻦ ﻧﻘﺺ اﻧﺠﺎم ﺷﺪ. ﻛﺎرﺑﺮد ﺑﺎﻟﻴﻨﻲ- ﺑﻜﺎر ﺑﺮدن ﭘﻼﺳﻤﺎي ﻏﻨﻲ ﺷﺪه ﺑﺎ ﭘﻼﻛﺖ ﺑﻪﻋﻨﻮان ﻳﻚ ﮔﺰﻳﻨﻪ ﻗﺎﺑﻞ دوام و زﻳﺴﺖ ﭘﺬﻳﺮ ﺑﺮاي ﺑﻬﺴﺎزي ﺗﺸﻜﻴﻞ ﺑﺎﻓﺖ اﻟﺘﻴﺎﻣﻲ و آﻣﺎدهﺳﺎزي زودﻫﻨﮕﺎم ﺑﺴﺘﺮ زﺧﻢ ﻗﺒﻞ از ﺟﺮاﺣﻲ ﺗﺮﻣﻴﻤﻲ ﺑﺎ ﺗﻜﻪاي از ﭘﻮﺳﺖ ﻣﻄﺮح ﻣﻲﺑﺎﺷﺪ. ﻫﻤﻪ ﺳﻪ ﻗﻼده ﮔﺮﺑﻪ ﺑﺎ ﭘﻼﺳﻤﺎي ﻏﻨﻲ ﺷﺪه ﺑﺎ ﭘﻼﻛﺖ اﺗﻮﻟﻮﮔﻮس ﺑﻪ ﻣﻨﻈﻮر اﻳﺠﺎد ﺑﺎﻓﺖ اﻟﺘﻴﺎﻣﻲ اوﻟﻴﻪ درﻣﺎن ﺷﺪﻧﺪ و ﺑﻌﺪ از آﻣﺎده ﺷﺪن ﺑﺴﺘﺮ ﻣﻨﺎﺳﺐ ﺗﺤﺖ ﻋﻤﻞ ﺟﺮاﺣﻲ ﺗﺮﻣﻴﻤﻲ ﻗﺮار ﮔﺮﻓﺘﻨﺪ.
