Abstract :
Objective- The objective was to assess local effect of brain derived neurotrophic factor (BDNF) on
functional recovery of peripheral nerve in rat sciatic nerve transection model.
Design- Experimental study.
Animals- Sixty male healthy white Wistar rats
Procedures- The animals were randomized into four experimental groups of 15 animals each: In shamoperated
group (SHAM), sciatic nerve was exposed and manipulated. In transected group (TC), left
sciatic nerve was transected and nerve cut ends were fixed in the adjacent muscle. In silicone graft group
(SIL) a 10-mm defect was made and bridged using a silicone tube. The graft was filled with phosphatedbuffer
saline alone. In treatment group the silicone tube (SIL/BDNF) was filled with 10 microliter BDNF
(100ng/kg BW). Each group was subdivided into three subgroups of five animals each and regenerated
nerve fibers were studied at 4, 8 and 12 weeks post operation.
Results- Behavioral testing, sciatic nerve functional study, gastrocnemius muscle mass and morphometric
indices showed earlier regeneration of axons in SIL/BDNF than in SIL group (p < 0.05).
Immunohistochemical study showed clearly more positive location of reactions to S-100 in SIL/BDNF
than in SIL group.
Conclusion and Clinical Relevance- When loaded in a silicone graft, BDNF improved functional
recovery and morphometric indices of sciatic nerve. This finding supports the role of BDNF after
peripheral nerve repair and may have clinical implications for the surgical management of patients after
nerve transection.

ﻫﺪف- ﻫﺪف از اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﺑﺮرﺳﻲ ﺗﺎﺛﻴﺮ ﺗﺰرﻳﻖ ﻣﻮﺿﻌﻲ ﻓﺎﻛﺘﻮر ﻧﻮروﺗﺮوﻓﻴﻚ ﻣﻐﺰي )BDNF( ﺗﻮام ﺑﺎ ﮔﺮاﻓﺖ ﺳﻴﻠﻴﻜﻮﻧﻲ ﺑﺮ روي ﺗﺮﻣﻴﻢ اﻋﺼﺎب ﻣﺤﻴﻄﻲ در ﻣﺪل ﻗﻄﻊ ﻋﺼﺐ ﺳﻴﺎﺗﻴﻚ رت ﺑﻮده اﺳﺖ.
ﻃﺮح- ﻣﻄﺎﻟﻌﻪ ﺗﺠﺮﺑﻲ
ﺣﻴﻮاﻧﺎت- 60 ﻗﻄﻌﻪ رت وﻳﺴﺘﺎر ﻧﺮ ﺳﺎﻟﻢ.
روش ﻛﺎر- رتﻫﺎ ﺑﻄﻮر ﺗﺼﺎدﻓﻲ ﺑﻪ ﭼﻬﺎر ﮔﺮوه 15 ﺗﺎﻳﻲ ﺗﻘﺴﻴﻢ ﺷﺪﻧﺪ. در ﮔﺮوه ﻛﻨﺘﺮل ﻧﺮﻣﺎل ) SHAM( ﻋﺼﺐ ﺳﻴﺎﺗﻴﻚ ﺳﻤﺖ ﭼـﭗ ﭘـﺲ از ﺑﺮش ﭘﻮﺳﺖ و ﻋﻀﻠﻪ ﺳﺮﻳﻨﻲ دﺳﺘﻜﺎري ﺷﺪه و ﭘﺲ از ﺧﻮﻧﺒﻨﺪي ﻣﻮﺿﻊ ﺑﺨﻴﻪ ﮔﺮدﻳﺪ.درﮔﺮوه ﻛﻨﺘﺮل ﻣﻨﻔﻲ )TC( ﭘـﺲ از دﺳـﺘﻴﺎﺑﻲ ﺑـﻪ ﻋﺼـﺐ ﺳﻴﺎﺗﻴﻚ ﺑﺎ ﻗﻄﻊ ﻋﺼﺐ، ﻧﻘﻴﺼﻪاي ﺑﻪ ﻃﻮل 10 ﻣﻴﻠﻴﻤﺘﺮ اﻳﺠﺎد ﮔﺮدﻳﺪ و اﻧﺘﻬﺎﻫﺎي ﻗﻄﻊ ﺷﺪه ﺑﻪ ﻋﻈﻼت ﻣﺠﺎور ﺑﺨﻴﻪ ﮔﺮدﻳﺪ. در ﮔـﺮوه ﺳـﻴﻠﻴﻜﻮن ﭘﺲ از اﻳﺠﺎد ﻧﻘﻴﺼﻪ 10 ﻣﻴﻠﻴﻤﺘﺮي، اﻧﺘﻬﺎﻫﺎي ﻗﻄﻊ ﺷﺪه ﭘﺮوﻛﺰﻳﻤﺎل و دﻳﺴﺘﺎل ﻋﺼﺐ ﺑﺎ اﺳﺘﻔﺎده از ﻟﻮﻟﻪ ﺳﻴﻠﻴﻜﻮﻧﻲ)SIL( ﺑﻪ ﻫﻢ ﻣﺮﺗﺒﻂ ﺷﺪﻧﺪ. در ﮔﺮوه درﻣﺎن )SIL/BDNF( ﭘﺲ از ﻛﺎرﮔﺬاري ﻟﻮﻟﻪ ﺳﻴﻠﻴﻜﻮﻧﻲ ﺑﻪ داﺧﻞ آن 10 ﻣﻴﻜﺮوﻟﻴﺘﺮ ﻣﺤﻠﻮل BDNF ﺑﺎ دز 1 ﻣﻴﻠﻲ ﮔﺮم ﺑﺮ ﻣﻴﻠﻲﻟﻴﺘﺮ ﺗﺰرﻳﻖ ﮔﺮدﻳﺪ. ﻫﺮ ﮔﺮوه ﻣﺘﻌﺎﻗﺒﺎ ﺑﻪ ﺳﻪ زﻳﺮ ﮔﺮوه 5 ﺗﺎﻳﻲ ﺗﻘﺴﻴﻢ ﮔﺮدﻳﺪه و در ﻣﻘﺎﻃﻊ زﻣﺎﻧﻲ 4، 8 و 12 ﻫﻔﺘﻪ ﺑﻌﺪ از ﺟﺮاﺣﻲ ﻣﻮرد ﻣﻄﺎﻟﻌـﻪ ﻗـﺮار ﮔﺮﻓﺘﻨﺪ.
ﻧﺘﺎﻳﺞ- ﺗﺴﺖﻫﺎي رﻓﺘﺎري و ﻋﻤﻠﻜﺮدي ﻋﺼﺐ ﺳﻴﺎﺗﻴﻚ و وزن ﻋﻀﻠﻪ دوﺑﻄﻨﻲ ﻧﺸﺎن داد ﻛﻪ ﻋﻤﻠﻜﺮد ﻋﺼﺐ ﺳﻴﺎﺗﻴﻚ در رتﻫﺎي درﻳﺎﻓﺖﻛﻨﻨﺪه ﮔﺮاﻓﺖ ﺳﻴﻠﻴﻜﻮﻧﻲ ﺑﻬﺒﻮدي ﻗﺎﺑﻞ ﻣﻼﺣﻈﻪاي ﭘﻴﺪا ﻛﺮده ﺑﻮد )P < 0.05(. ﺷﺎﺧﺺﻫﺎي ﻣﻮرﻓﻮﻣﺘﺮﻳﻚ و ارزﻳﺎﺑﻲﻫﺎي اﻳﻤﻨﻮﻫﻴﺴﺘﻮﺷﻴﻤﻴﺎﻳﻲ ﺑﻴﺎﻧﮕﺮ ﺗﺮﻣﻴﻢ ﻗﺎﺑﻞ ﻣﻼﺣﻈﻪاي در ﮔﺮوه درﻣﺎن در ﻣﻘﺎﻳﺴﻪ ﺑﺎ ﮔﺮوه ﺷﺎﻫﺪ ﺑﻮد 0.05 P < (.
ﻧﺘﻴﺠﻪ ﮔﻴﺮي و ﻛﺎرﺑﺮد ﺑﺎﻟﻴﻨﻲ- ﻛﺎرﺑﺮد ﻣﻮﺿﻌﻲ BDNF ﻋﻤﻠﻜﺮد ﻋﺼﺐ ﺳـﻴﺎﺗﻴﻚ و ﺷـﺎﺧﺺ ﻫـﺎي ﻣﻮرﻓﻮﻣﺘﺮﻳـﻚ ﻋﺼـﺒﻲ را ﺑﻬﺒـﻮد ﺑﺨﺸـﻴﺪ ﻣﻲﺗﻮان از آﻧﻬﺎ ﺑﻪ ﻋﻨﻮان درﻣﺎن ﻣﻮﺛﺮ در ﺗﺮﻣﻴﻢ ﺟﺮاﺣﺎت اﻋﺼﺎب ﻣﺤﻴﻄﻲ اﺳﺘﻔﺎده ﻛﺮد.
