Abstract :
Objective: To assess the efficiency and fertility potential after two different modalities for treatment of undisturbed tubal pregnancy. Methods: A total number of 334 cases were diagnosed as undisturbed ectopic pregnancy and were managed by laparoscopic surgery, salpingotomy (n=182) and salpingctomy (n=152). Immediate, delayed follow-up and assessment of future fertility was done. Results: There was no statistically significant difference between both groups as regard conversion to laparotomy. There was highly statistically significant difference between the two groups as regard repeated laparoscopy with salpingectomy for persistent trophoblastic disease. There was statistically significant prolonged operative time in salpingectomy group (51.9±15.3) vs (35±5.2) in salpingotomy group. There was statistically significant high fertility outcome and law recurrence of ectopic at ipsilateral tube in salpingectomy group but there was no statistical significant difference between both groups as regard recurrence of ectopic on contralateral tube. Conclusion: Laparoscopic management whether conservative (salpingotomy) or radical (salpingectomy) is still aquestion. Management by salpingostomy carries a risk of persistent trophoblast and a risk of recurrence in ipsilateral tube.
