Abstract :
Thisstudyisaimedatdetermining(1) theeffectofroot-endresection,ultrasonicroot-endpreparation, androot-endfillingontheincidenceofcrackformationandpropagationbyusingadigitalmicroscope (DM) andopticalcoherencetomography(OCT) and (2) theperformanceofOCTonthedetectionofcracksbycomparingwithmicrocomputedtomography(micro-CT) asareferencestandard.Methodology.Thirtyextractedlowerincisorswereendodonticallytreatedandsubjectedtoroot-endresectionandultrasonicroot-endcavitypreparation. Then,theteethweredividedintothreegroups(n=10,each), andtheroot-endcavitywaseitherleftunfilledorfilledwithmineraltrioxideaggregate (MTA)orsuper-EBA. TheresectedsurfacewasobservedwithOCTandDMaftertheroot-endresection,ultrasonicroot-endpreparation,androot-endfilling,andthefrequencyofincompleteandcompletecrackswererecorded.Theobservationwasrepeatedafter twoweeks,onemonth,andtwomonths,andmicro-CT scansaftertwomonthsweretakenasthegoldstandard.Results. TheDMresultsshowdentinalcrackformationin47% of thesamplesfollowingroot-endresectionandin87%followingultrasonicpreparation. Aftertheultrasonicpreparation,noexistingcrackpropagatedtoacompletecrack, butnewcrackswereformed. MTAandsuper-EBAhadnoeffectoncrackformation.TheSpearmancorrelationcoefficientbetweenOCTandDMwas0.186(veryweakcorrelation;p=0:015). Sensitivityandspecificityincomparisontomicro-CTwere0.50and0.55inOCTand1.00and0.35inDM,respectively.McNemar’stestshowedasignificantdifferencebetweenOCTandDM(p<0:05). Conclusion.Apicalresectionandultrasonicpreparationcouldformdentinalcracks.OCTandDMshoweddifferentdetectionfrequenciesofcrackswithveryweakcorrelation.DMshowedsuperiorsensitivitycomparedwithOCT.
