Abstract :
opresenta digital method that combinesintraoralandfacescanning for the computer-assisteddesign/computer-assistedmanufacturing (CAD/CAM) fabricationofimplant-supportedbarsformaxillaryoverdentures.Methods. Overa2-yearperiod,allpatientspresentingtoaprivatedentalclinicwitharemovablecompletedentureinthemaxilla,seekingrehabilitationwithimplants,wereconsideredforinclusioninthisstudy. Inclusioncriteriawerefullyedentulousmaxilla,functionalproblemswiththepreexistingdenture,opposingdentition,and sufficientbonevolumetoinsertfourimplants.Exclusioncriteriawereage<55years, needforboneaugmentation,uncompensateddiabetesmellitus,immunocompromisedstatus,radio-and/orchemotherapy,andprevioustreatmentwithoraland/orintravenousaminobisphosphonates.AllpatientswererehabilitatedwithamaxillaryoverdenturesupportedbyaCAD/CAMpolyether-ether-ketone (PEEK) implant-supportedbar. Theoutcomesofthestudywerethepassivefit/adaptationofthebar, the1-yearimplantsurvival,andthesuccessratesoftheimplant-supportedoverdentures. Results.15patients (6males,9females;meanage688±47years) received60implantsandwererehabilitatedwithamaxillaryoverdenturesupportedbyaPEEKbar,designedandmilledfromanintraoraldigitalimpression. Theintraoralscanswereintegratedwithfacescans,inordertodesigneachbarwithallavailablepatientdata(softtissues,prosthesis,implants,andface)inthecorrectspatialposition.Whentestingthe3D-printedresinbar,12barsoutof15 (80%) hadaperfectpassiveadaptationandfit;incontrast,3outof15 (20%) didnothaveasufficientpassivefitoradaptation.Noimplantswerelost,fora1-yearsurvivalof100%(60/60survivingimplants).However,somecomplications(twofixtureswithperi-implantitisinthesamepatientandtworepairedoverdenturesintwodifferentpatients)occurred.Thisdetermineda1-yearsuccessrateof80%for theimplant-supportedoverdenture. Conclusions.Inthisstudy,thecombinationofintraoralandfacescansallowedtosuccessfullyrestorefullyedentulouspatientswithmaxillaryoverdenturessupportedby4implantsanda CAD/CAMPEEKbar. Furtherstudiesareneededtoconfirmtheseoutcomes
