  • Abstract
    The study aimed to determine the success rate, surgical characteristics, and complications of bilateral endoscopic retroperitoneal lumbar sympathectomy in a unique operation for the treatment of plantar hyperhidrosis. From January 2022 to January 2024, a cross-sectional study was conducted on 81 patients with plantar hyperhidrosis treated with bilateral endoscopic retroperitoneal lumbar sympathectomy in a unique operation. Surgical factors were evaluated during and after the operation. All cases were successfully treated with bilateral endoscopic retroperitoneal lumbar sympathectomy in a unique operation. Pathological results confirmed 100% sympathetic nerve tissue. The average operation time was 88.4 ± 25.1 minutes. The average blood loss was 10.6 ± 4.7 ml. There was no significant statistical difference in blood loss between the right and left sides. Sympathetic chains were cut at L2-L3 and L3-L4 levels. There was a statistically significant difference between the right and left sides with p 0.001. Complications included intraoperative peritoneal tears, accounting for 4.9% and classified as grade 1 according to Clavien-Dindo classification. This complication resolved on its own without specific treatment. Postoperative pain levels were as follows: 37% of patients had no pain, 29.6% had pain level 1, 29.6% had pain level 2, and 3.7% had pain level 3. All surgical wounds healed well. Bilateral endoscopic retroperitoneal lumbar sympathectomy in a unique operation for the treatment of plantar hyperhidrosis is safe and effective.
