Abstract :
Purpoe: To compare the effect of extracapular cataract extraction with poterior chamber intraocular len (ECCE/PC-IOL) v intracapular cataract extraction with aphakic glae (ICCEAG) on everyday viual functioning and quality of life. Method: In a nonmaked randomized controlled clinical trial, 3,400 bilateral viion-impaired patient, aged 40 to 75 year, with operable cataract were randomly aigned to receive one of the two treatment option. One half in each group were randomly elected for interviewer adminitration of viual functioning and quality of life quetionnaire before urgery and at 6 and 12 month after urgery. Reult: Both ICCE-AG and ECCE/PC-IOL produced dramatic improvement in viual functioning and quality of life core. Patient receiving ECCE/PC-IOL reported larger beneficial change than did thoe receiving ICCE-AG, compatible with additional beneficial effect of a moderate magnitude for viual functioning and of a maller beneficial magnitude for quality of life. All between-group difference were highly tatitically ignificant (P< .00001). The additional benefit of ECCE/PC-IOL are not explained by viual acuity difference. A higher proportion of patient in the ICCE-AG group reported problem on a viion problem checklit at 6 month (more than 50%) than did patient in the ECCE/PC-IOL group (approximately 30%). Concluion: In thi developing-country etting, ICCE-AG and ECCE/PC-IOL were aociated with ubtantial benefit in improved everyday viion function and viion-related quality of life. Patient who received ECCE/PC-IOL reported greater benefit and fewer problem with viion than did patient who received ICCE-AG.
