Abstract :
Purpoe
To decribe dene vitriti a the primary manifetation of ocular yphili in three human immunodeficiency viru (HIV)-poitive patient and to determine the repone of thee patient to the etablihed regimen for neuroyphili.
Method
Anti-Toxoplama gondii IgM and IgG antibody titer, tuberculin kin tet, chet radiograph, and erum angiotenin-converting enzyme level were obtained becaue tuberculoi, arcoidoi, and toxoplamoi were in the differential diagnoi. Two of the three patient were not known to have HIV infection at the time of initial examination and conented to HIV teting. Treponemal and nontreponemal tet were performed on erum and cerebropinal fluid to etablih a definitive diagnoi. Treatment for neuroyphili wa initiated, and daily ophthalmic examination were performed, with careful attention to ign commonly aociated with yphilitic eye dieae.
Reult
All three patient exhibited improvement in viual acuity and reolution of vitreou haze. There wa no evidence of other ign of poterior uveiti. The one patient for whom there ha been a 6-month follow-up howed no equelae of hi eye dieae.
Concluion
Human immunodeficiency viru-poitive patient with yphili may preent atypically dene vitriti. In thee patient, vitriti may be the firt manifetation of yphili. The regimen for neuroyphili provide effective therapy. Moreover, in ome patient, yphilitic vitriti may be the initial manifetation of HIV dieae.
