Abstract :
Purpoe: To determine the prevalence of and rik factor for uperior egmental optic nerve hypoplaia in offpring of mother with type I diabete mellitu. Method: Thirty-four ubject aged between 4 and 37 year, the children of 23 mother who had type I diabete mellitu at the time of pregnancy, aged between 4 and 37 year, were recruited from one intitution and propectively examined in earch of an optic dik–related anomaly. Reult: Of the 34 ubject, three female (8.8%) howed claic ophthalmocopic and perimetric feature of uperior egmental optic nerve hypoplaia, bilateral in two patient and unilateral in one. Pregnancie leading to affected children howed a tendency to be horter, birth weight to be lower, and control of maternal diabete mellitu to be poorer compared with pregnancie reulting in unaffected children. No variable unique to the affected ubject a oppoed to the unaffected majority could be identified. Concluion: We found a uperior egmental optic nerve hypoplaia, decribed a a “tople dik,” in three of 34 ubject (8.8%) at rik for thi condition. Tople dik thu eem to be more common than wa previouly thought, poibly having been mied becaue of it ubtle ign and only mild impairment of viual performance in affected individual. Female ex, hort getation time, low birth weight, and poor maternal diabete control may repreent additional rik factor for the development of a tople dik. It pathogenei remain obcure, but the reponible pathogenic event may occur in the perinatal period.
