Abstract :
Purpoe: To report a urgical method for tranlocation of the foveal retina in eye with ubfoveal choroidal neovacularization. Method: In three eye of three patient, a crecent-haped, partial-thickne cleral reection wa performed near the equator at either the uperotemporal or the inferotemporal quadrant. A near-total retinal detachment wa created; then the edge of the reected clera were utured, cauing hortening of the clera with ubequent reattachment of the retina, reulting in tranlocation of the fovea to an area overlying nonfoveal retinal pigment epithelium and choroid. Reult: In three eye of three patient, the fovea wa urgically tranlocated to overlie retinal pigment epithelium that preoperatively wa not underlying the fovea. In two patient, laer photocoagulation wa applied to the choroidal neovacularization that, after tranlocation of the fovea, wa no longer ubfoveal, o that the photocoagulation wa not aociated with immediate viual lo. After a follow-up of 4 to 6 month, the viual acuity had improved in all patient (from 20/126 preoperatively to 20/70 in one patient, from 20/200 preoperatively to 20/70 in the econd, and from 20/160 to 20/30 in the third). The patient noted ditortion or tilting of the image, which improved over time. Concluion: Limited foveal tranlocation may offer a therapeutic modality to preerve or improve viion in cae of ubfoveal choroidal neovacularization. Additional follow-up i needed to ae the impact of potential complication aociated with the urgical procedure, uch a retinal detachment, proliferative vitreoretinopathy, and cataract, a well a the poibility of recurrent choroidal neovacularization.
