Abstract :
PURPOE: To invetigate the ocular pharmacokinetic and efficacy of oral trovafloxacin, a novel fluoroquinolone antibiotic, in taphylococcu epidermidi endophthalmiti. METHOD: Albino rabbit (n = 20) were infected with an intravitreal inoculum of epidermidi (1.0 × l08 colony-forming unit [CFU]/0.1 ml) and 24 hour later received a ingle oral doe of trovafloxacin (250 mg/kg). erum and intraocular ample from infected and control (noninfected) eye were obtained up to 24 hour after antibiotic adminitration for meaurement of trovafloxacin level. A econd group of rabbit (n = 72) wa infected intraocularly and randomized 24 hour later to oral trovafloxacin (250 mg/kg twice a day) for 6 day or no treatment (control). Treatment efficacy wa aeed by vitreou culture, clinical examination, and hitopathology. REULT: Following a ingle doe of trovafloxacin, maximal vitreou level were achieved at 8 hour in infected eye, with a penetration ratio of 36%. Vitreou level were greater than 15 time the minimum inhibitory concentration of the train employed. In animal with etablihed endophthalmiti, treated eye were terilized after 5 day (P = .0495) compared with control eye, which autoterilized at 14 day. Clinical and hitologic examination revealed ignificant amelioration of anterior egment inflammation in treated eye, although evere detruction of poterior egment tructure occurred in both group after 6 day of therapy. CONCLUION: Thee data upport trovafloxacin a a potential oral agent for treatment or prophylaxi of epidermidi endophthalmiti, although retinal alteration that occur over the period required for vitreou terilization ugget that it will not replace intravitreal therapy in etablihed endophthalmiti.
