Abstract :
PURPOE: To determine whether ilicone material ued in retinal detachment repair and cataract urgery increae erum IgG binding to ilicone and identify correlation with complication of ocular urgery. METHOD: erum from 49 patient who had ocular urgery uing ilicone material wa examined. Patient group included cleral buckling (n = 25), ilicone oil tamponade (n = 3), cleral buckling and ilicone oil tamponade (n = 9), and ilicone len implant after cataract extraction (n = 12). Convalecent ample for all patient and preoperative ample from 19 patient (18 cleral buckling and one ilicone oil tamponade) were examined. Potoperative complication were monitored for up to 108 month (mean, 10.7 month; mode, 1.5 month; range, 1 to 108 month). ample were evaluated for the extent of IgG binding to ilicone uing a micromodification of a previouly decribed enzyme-linked immunoorbent aay method. REULT: In 19 patient, IgG binding level in preoperative ample were 21 arbitrary unit (AU) or le. Of the 25 buckling patient, one developed complication; however, in all patient the potoperative level of IgG binding to ilicone were low (2.2 to 20.0 AU). Although four ilicone len patient developed mild complication, none diplayed potoperative IgG binding level of greater than 20 AU. Three patient who underwent both cleral buckling and ilicone oil tamponade developed complication; one of thee patient, who wa alo noted to have ytemic connective tiue dieae, had a ignificant elevation in potoperative erum IgG binding to ilicone. CONCLUION: tatitically ignificant elevation of erum IgG binding to ilicone were noted potoperatively in only one patient who had a ytemic connective tiue dieae. The complication rate and frequency of enhanced erum IgG binding to ilicone wa low, making correlation to urgical complication difficult. Examination of matched ample uggeted that if ocular expoure to ilicone implant enhance the level of erum IgG binding to ilicone, it mut be a rare event that hould not alter the clinical ue of thee important device.
