  • Abstract
    PURPOE: To invetigate the characteritic of peripapillary atrophy in primary angle-cloure glaucoma and to compare peripapillary atrophy in primary angle-cloure glaucoma with that in primary open-angle glaucoma. METHOD: Twenty-one eye (of 21 patient) with chronic primary angle-cloure glaucoma that had no evidence of an acute attack or combined-mechanim glaucoma and 31 eye (31 patient) with primary open-angle glaucoma were enrolled in thi tudy. The cup-to-dik area ratio and the viual field mean deviation were matched in both group. The prevalence of peripapillary atrophy and peripapillary atrophy-to-dik area ratio of eye with primary angle-cloure glaucoma and eye with primary open-angle glaucoma were compared. The correlation between the peripapillary atrophy area and the cup-to-dik area ratio or the mean deviation wa alo evaluated. REULT: Eight (38%) of 21 eye with primary angle-cloure glaucoma and 21 (68%) of 31 eye with primary open-angle glaucoma had peripapillary atrophy. The peripapillary atrophy-to-dik area ratio averaged 0.16 in primary angle-cloure glaucoma and 0.41 in primary open-angle glaucoma. There were tatitically ignificant difference regarding the prevalence of peripapillary atrophy (P = .048) and peripapillary atrophy-to-dik area ratio (P = .005) between the two group. There wa no ignificant correlation between the peripapillary atrophy area and the cup-to-dik area ratio or the mean deviation in primary angle-cloure glaucoma. In contrat, the peripapillary atrophy-to-dik area ratio ignificantly correlated with the cup-to-dik area ratio (P = .003) and the mean deviation (P = .0001) in primary open-angle glaucoma. CONCLUION: The peripapillary atrophy in primary angle-cloure glaucoma ha a different relationhip to the tructural and functional optic dik change than that in primary open-angle glaucoma. Different mechanim eem to be involved in the development of the optic dik damage in the two type of glaucoma
