Retinal detachment in phakic eye with anterior chamber intraocular lene to correct evere myopia Original Reearch Article

PURPOE: To analyze the incidence and characteritic of retinal detachment in patient with evere myopia corrected by implantation of phakic anterior chamber intraocular lene. METHOD: We tudied retinal detachment in 166 conecutive eye (98 patient) that underwent implantation of angle-upported phakic anterior chamber intraocular lene (model ZB5M and ZB5MF; Domilen; Lyon, France) for the correction of evere myopia (follow-up ± D, 45.26 ± 14.65 month; range, 20 to 84 month). REULT: Retinal detachment occurred in eight eye (4.8%); four eye belonged to men and four to women. The time between implanting urgery and retinal detachment wa 17.43 ± 16.4 month (range, 1 to 44 month). In all cae, retinal detachment wa pontaneou. In even eye, the retina wa reattached uccefully during the firt retinal detachment urgery. Mean bet-corrected viual acuity after phakic anterior chamber intraocular len implantation and before retinal detachment development wa 20/50 (range, 20/100 to 20/25). After retinal detachment repair, bet-corrected viual acuity wa 20/73 (range, 20/2000 to 20/33). In thee even eye, difference between bet-corrected viual acuity before and after reattachment were not tatitically ignificant (P = .898, paired tudent t tet). In one eye, a proliferative vitreoretinopathy wa oberved, which required additional treatment by vitrectomy and explantation of the phakic anterior chamber intraocular len. A refractive change wa oberved after retinal detachment repair, from −1.1 ± 0.7 diopter (range, 0.00 to –2.50 diopter) before retinal detachment and –2.8 ± 1.1 diopter (range, −1.00 to –4.50 diopter) after retinal detachment urgery (P = .03, paired tudent t tet). CONCLUION: The implantation of a phakic anterior chamber intraocular len a a correcting procedure for evere myopia wa followed by a 4.8% incidence of retinal detachment. Conventional cleral urgery wa ucceful in mot cae, without cauing ignificant change in the final bet-corrected viual acuity. A ignificant increae in the myopic pherical equivalent wa oberved after retinal detachment repair in thee patient.
