Abstract :
PURPOE: To decribe intentional placement of intraocular len haptic in the ciliary ulcu of patient with uveiti who are at high rik for potoperative poterior ynechiae and len dilocation. METHOD: We reviewed our experience with 16 eye of 12 patient with uveiti who underwent cataract urgery with ciliary ulcu fixation of intraocular lene. Patient were followed for a median of 16.5 month (range, 9 to 44 month) after urgery. We evaluated eye for urgical technique and the following preoperative and potoperative factor: bet-corrected viual acuity, intraocular preure, anterior chamber cell, and poterior ynechiae. The following additional potoperative factor were ought: len dilocation, len edge capture, and evidence of pigment diperion. REULT: Poterior ynechiae were preent in 13 eye before urgery; potoperative poterior ynechiae developed in only three of thee eye. Thee adheion reulted in len edge capture in one eye and limited len decentration in another. cant pigment wa preent on the len optic or in the anterior chamber, uggeting pigment diperion, in four eye. We found no evidence of conitently increaed anterior egment inflammation or intraocular preure after urgery when compared with preoperative level for thi group of patient. Potoperative poterior ynechiae were een more often in eye that had can-opener anterior capulotomy than in eye that had continuou, curvilinear capulorhexi (P = .036). CONCLUION: Ciliary ulcu fixation allow the intraocular len to erve a a phyical barrier between the iri and the len capule remnant. Thi technique may be ueful for reducing the rik of potoperative poterior ynechiae in patient with uveiti without increaing the rik of other potoperative problem.
