Abstract :
PURPOE:
To decribe the occurrence of unilateral retinal hemorrhage in four cae of documented child abue, including a cae in which retinal hemorrhage were an incidental finding on routine examination.
METHOD:
Cae report.
REULT:
Three children, 5 to 17 month of age, with upected child abue had fundu examination with a dilated pupil a part of their evaluation. An additional child, 6 month of age, received fundu examination with a dilated pupil a part of follow-up for regreed retinopathy of prematurity. Each of the four children had extenive retinal or preretinal hemorrhage in one eye only. Three of the four had ecchymoe on the ipilateral face or neck. Two had evidence of bone fracture on keletal urvey. All four had neuroimaging that documented cerebral hemorrhage or infarct. In all four cae an adult caretaker admitted haking, choking, or queezing the child. One child died. Two had reolution of retinal hemorrhage, wherea one required vitrectomy. All three had at leat partial recovery of viion in the affected eye after amblyopia treatment.
CONCLUION:
In cae of documented child abue, unilateral retinal or preretinal hemorrhage may be preent. Ophthalmologit hould recognize that unilateral retinal or preretinal hemorrhage may be aociated with child abue.
