Abstract :
PURPOE: To decribe mechanim and injury characteritic influencing viual outcome in eye with open-globe injurie caued by deadly weapon and to apply the claification ytem introduced by the Ocular Trauma Claification Group. METHOD: Two-hundred-twenty-eight eye of 212 conecutive patient, who were motly injured in military confrontation, were analyzed. Mechanim and injury characteritic were evaluated for predicting viual outcome according to the recently tudied claification ytem a well a other variable pertinent to thi pecific clinical etting of evere eye trauma. Final viual acuitie were defined a favorable (5/200 or better) or unfavorable (le than 5/200, including enucleation). REULT: The mean age of patient wa 23 year, and the mean follow-up wa 5.7 month. The predictor for favorable viual outcome were type B, grade 1, zone I, and relative afferent pupillary defect-negative injurie. The predictor for unfavorable outcome were type A, grade 5, zone III, and relative afferent pupillary defect-poitive injurie. Land mine and hand grenade injurie had the wort outcome among cauative agent. Proliferative vitreoretinopathy, compriing 30.4% of potoperative failure, wa the mot common complication. CONCLUION: Deadly weapon–related open-globe injurie, epecially thoe aociated with land mine and hand grenade, have devatating viual reult. Evaluation of trauma mechanim and injury characteritic according to the Ocular Trauma Claification ytem eem to predict viual outcome in thi erie of evere open-globe injurie.
