Abstract :
To clarify the pathologic change of the detached neuroenory retina in rhegmatogenou retinal detachment. METHOD: Retinal image were propectively examined by optical coherence tomography in 25 eye of 25 conecutive patient with rhegmatogenou retinal detachment. We excluded the patient whoe retinal detachment did not involve the central fovea or patient with poor fixation during optical coherence tomography (OCT) examination. Optical coherence tomography wa canned through the center of the fovea. The patient ranged in age from 15 to 77 year (mean, 45 year; D, 20 year). The period from onet of ubjective ymptom of retinal detachment to OCT ranged from 2 to 60 day (mean, 16 day; D, 18 day). Optical coherence tomography finding, bet-corrected viual acuity, and the height of the retinal detachment at the central fovea were tatitically analyzed uing ANCOVA (analyi of covariance) and the Mann-Whitney U tet. REULT: In 25 eye of 25 patient, OCT of the detached neuroenory retina at and adjacent to the center of the fovea demontrated normal retinal tructure (10 eye, 40%), intraretinal eparation (7 eye, 28%), and an undulated eparated outer retina (8 eye, 32%). Three tatitically ignificant factor affected bet-corrected viual acuity: intraretinal eparation (P = .001), intraretinal eparation with undulated outer retina (P = .001), and height of retinal detachment at the central fovea (P< .001). Bet-corrected viual acuity wa ignificantly wore in the 15 eye with intraretinal eparation with or without an undulated outer retina than in the 10 eye with retinal thickening but no intraretinal eparation (P = .036). The eight eye with undulated eparated outer retina howed ignificantly higher retinal detachment at the central fovea than the even eye with intraretinal eparation but no undulated outer retina (P = .009) and the 10 eye without intraretinal eparation (P = .016). The duration from onet of ubjective ymptom to OCT wa not related to the occurrence of intraretinal eparation of the detached retina. CONCLUION: Intraretinal eparation of the detached retina occurred frequently and hortly after retinal detachment in rhegmatogenou retinal detachment and wa one of the factor aociated with poor viion in rhegmatogenou retinal detachment. Bet-corrected viual acuity ignificantly decreaed in the highly detached retina.
