Phacoemulification and len implantation after cleral buckling urgery

PURPOE: To determine the intraoperative and potoperative complication and bet-corrected viual acuity outcome of eye undergoing phacoemulification and intraocular len implantation after retinal detachment repair by the cleral buckling technique. METHOD: The chart of all patient who underwent phacoemulification and intraocular len implantation between July 1991 and May 1998 in two urgical practice were reviewed to identify eye with a hitory of retinal detachment repaired by the cleral buckling technique. Eye with a hitory of par plana vitrectomy were excluded. Demographic and urgical data, preoperative and potoperative bet-corrected viual acuity, and intraoperative and potoperative complication were recorded. REULT: We identified 34 eye of 32 patient. The mean interval from retinal detachment repair to phacoemulification wa 12.4 year. The mean interval from phacoemulification to final examination wa 20 month. Rik factor for retinal detachment included iolated myopia (82%), myopia with lattice retinal degeneration (5.9%), and myopia with trauma (8.8%). One eye (2.9%) had no identifiable rik factor. Final bet-corrected viual acuity of 20/40 or better wa attained in 29 (85%) of 34 eye and 20/20 or better in 18 (53%) of the eye. Of the five eye with the lowet bet-corrected viual acuity, three had a macula-off retinal detachment; one had a poterior capule opacity, epiretinal membrane, and corneal edema econdary to ocular ichemia; and one had advanced glaucoma. All five eye till experienced an improvement in bet-corrected viual acuity. With regard to complication, one eye had a poterior capular tear with vitreou lo and another developed a potoperative retinal tear. Poterior capule opacification requiring laer capulotomy developed in 13 eye (38%). No eye developed a retinal redetachment. CONCLUION: Phacoemulification and intraocular len implantation can be performed afely after cleral buckling urgery and excellent bet-corrected viual acuity reult can be attained in mot eye. No modification of urgical technique i neceary. No retinal redetachment occurred in thi erie.
