Abstract :
PURPOE: To preent an example of a pattern of line reembling fine lattice on the corneal urface ubequent to laer in itu keratomileui (LAIK). Thi ubtle phenomenon may be relatively common and may affect viual outcome. METHOD: Cae report. REULT: A 41-year-old year old man with high myopia and bet-corrected viual acuity of 20/20 +2 in each eye underwent laer in itu keratomileui (LAIK). No operative or potoperative complication occurred. No triae were evident on lit-lamp examination with direct illumination and retroillumination at the time of urgery or in the potoperative period. Potoperative uncorrected viual acuity wa 20/25 with a bet-corrected pectacle correction of 20/25 in both eye. Fine line in a lattice pattern were een only with fluorecein dye in the precorneal tear film a area of “negative tain” within the LAIK flap. With tear film upplementation, the line were le evident and viual acuity improved. One year potoperatively, hi uncorrected viual acuity wa 20/25 in both eye. The bet-corrected pectacle viual acuity wa RE: 20/20 −2, LE: 20/25. The fine line were till preent within the flap. A oft contact len improved viual acuity to 20/20 in both eye. Although all four puncta were occluded, he had no epiphora. CONCLUION: Fine line in a lattice pattern that may repreent fold in the epithelium or Bowman layer may be preent within the flap after LAIK and may adverely affect viual acuity. They may be viible a area of negative tain with fluorecein dye in the precorneal tear film in the abence of any triae viible in the flap. Thee uperficial line have been een more in patient with high degree of correction and in patient with dry eye. If viual acuity i affected, it may be improved with punctal occluion, tear upplement, or a contact len.
