Abstract :
PURPOE: To report a model of choroidal neovacularization feeder veel that reconcile current hitologic, angiographic, and clinical data, and to report experimental tudie that invetigate the potential of indocyanine-green-dye–enhanced photocoagulation to improve feeder-veel treatment. METHOD: A model of choroidal neovacularization feeder veel wa conceived to account for current hitologic and angiographic data. Baed on that model, experimental tudie of the efficacy of indocyanine green-dye–enhanced photocoagulation were performed, uing pigmented rabbit eye a a model ytem. A Zei fundu camera wa modified to permit viualization of choroidal blood flow by high-peed indocyanine green angiography and to permit imultaneou delivery of 810-nm-wavelength diode laer photocoagulation pule to pecific choroidal vacular target during indocyanine green-dye bolu tranit. REULT: Choroidal neovacularization feeder veel appear to originate in the attler layer (that i, that portion of the choroidal vaculature coniting of medium-diameter veel) and enter the choriocapillari in cloe proximity to the mall capillary-like veel that penetrate Bruch membrane and communicate with the choroidal neovacularization. The rabbit eye experiment demontrated that the preence of high indocyanine green dye concentration in circulating blood enhance uptake of near-infrared laer energy (three eye); injection of equential indocyanine green dye bolue reult in gradually decreaed efficiency of dye-enhanced photocoagulation (two eye); and by application of laer energy during the initial tranit of mall-volume, high-concentration indocyanine green dye bolue, dye-enhanced photocoagulation of large diameter choroidal arterie can be accomplihed with relatively little concomitant retinal tiue damage (three eye). CONCLUION: Although future trial will be neceary to ubtantiate thee initial finding in the clinical arena, it appear that the efficiency of choroidal neovacularization feeder-veel photocoagulation may be enhanced, while minimizing concomitant damage to overlying retinal tiue, by delivery of 810-nm wavelength laer energy immediately upon arrival of a high-concentration indocyanine green dye bolu in a targeted feeder veel. However, molecule of dye adhering to veel wall or lying in tiue intertitial pace appear to divert laer energy from the photocoagulation proce, o efficiency of indocyanine green dye–enhanced photocoagulation gradually diminihe a the number of injected dye bolue increae.
