Abstract :
PURPOE: To report a cae of Klebiella ubretinal abce with a ucceful viual outcome with treatment and to review the literature pertaining to focal intraocular infection in bacterial endophthalmiti.
METHOD: Clinical data including medical hitory, finding on phyical examination, blood culture, and an abdominal computed tomographic can were collected in a 32-year-old man with Klebiella epi, liver abcee, and a focal ubretinal abce. Ocular data including viual acuity, fundu photograph, fluorecein angiography, and ultraound were evaluated, a were reult of culture and hitopathologic tudie.
REULT: Depite immediate intervention, including vitreou tap and intravitreal antibiotic, the eye deteriorated, with enlargement of the abce. A par plana vitrectomy wa performed in which the ubretinal abce material wa removed after an extenive retinectomy of the involved area in aociation with an endophotocoagulative barrier and intravitreal amikacin without ga or oil tamponade. Culture confirmed Klebiella ubretinal infection. A retinal detachment occurred 1 month potoperatively and wa uccefully repaired. Viual acuity wa 20/30 and ha remained table for 14 month.
CONCLUION: Klebiella endophthalmiti with ubretinal abce formation i a rare but devatating ocular condition. In the preent cae, prompt intervention with extenive retinectomy, complete abce exciion, and intravitreal antibiotic therapy reulted in unprecedented viual recovery.
