Abstract
PURPOE: To evaluate the afety and efficacy of photorefractive keratectomy for the treatment of primary compound myopic atigmatim.
METHOD: In a propective tudy, 93 eye from 56 patient with a mean pherical equivalent of −4.98 ± 1.80 diopter (range, −1.75 to −8.5) underwent photoatigmatic refractive keratectomy with the ummit Apex Plu excimer laer uing erodible mak technology and were followed for 2 year. Primary outcome meaure included an aement of atigmatic correction through vector analyi, manifet refraction, uncorrected viual acuity, corneal clarity, and the preence of advere ymptom.
REULT: Eighty-five eye (91.4%) were available for analyi at 6 month. Mean pherical equivalent refraction wa reduced 85% (mean, −0.75 ± 0.85 diopter) and the target-induced atigmatim wa reduced 70% (mean, 0.98 ± 1.88 diopter). Forty-eight eye (56%) had an uncorrected viual acuity of 20/20 or greater, wherea 70 eye (82%) had an uncorrected viual acuity of 20/40 or greater. Twenty-four eye (26% ) required re-treatment becaue of undercorrection of the pherical equivalent and atigmatic component after the 6-month follow-up. Fifty-nine of the remaining eye were available at the 24-month viit. Mean pherical equivalent refraction wa reduced to −0.39 ± 0.72 diopter (91.8%). The target-induced atigmatim wa reduced 64% from 1.74 diopter. Forty-one eye (81.3%) were within ±1.0 diopter of attempted pherical equivalent correction. tability within a pherical equivalent of ±0.5 diopter occurred after the firt potoperative month. Fifty-ix eye (94.9%) had an uncorrected viual acuity of 20/40 or greater, wherea 34 eye (57.6 %) demontrated an uncorrected viual acuity of 20/20 or greater. One eye (1.7%) lot 2 or more line of bet pectacle-corrected viual acuity.
CONCLUION: Photoatigmatic refractive keratectomy with the ummit Apex Plu excimer laer i a afe and effective method of reducing compound myopic atigmatim. However, higher re-treatment rate may reult from ignificant undercorrection becaue of current laer algorithm and variability in the mean angle of error.
