Abstract :
PURPOE: To identify and decribe long-term poterior and anterior egment complication of immune recovery uveiti in patient with inactive cytomegaloviru retiniti who are undergoing highly active antiretroviral therapy–mediated recovery of immune function. METHOD: A propective cohort tudy at a univerity medical center. Twenty-nine eye of 21 patient with immune recovery uveiti and inactive cytomegaloviru retiniti were followed for 14.5 to 116 week (median, 43 week) after diagnoi of immune recovery uveiti. REULT: Nine eye of nine patient developed viually important complication involving the poterior egment, anterior egment, or a combination of both. Poterior egment complication included evere proliferative vitreoretinopathy in three eye and pontaneou vitreou hemorrhage from avulion of a blood veel econdary to contraction of the inflamed vitreou in one eye. Proliferative vitreoretinopathy recurred in all cae after urgery, everely compromiing the viual outcome. Anterior egment complication included poterior ubcapular cataract with viion decreae in five eye and peritent anterior chamber inflammation after cataract extraction, reulting in poterior ynechiae and large viually important len depoit in three eye. CONCLUION: Peritent inflammation in immune recovery uveiti may lead to viion-threatening complication, uch a proliferative vitreoretinopathy, poterior ubcapular cataract, and evere potoperative inflammation. Immune recovery uveiti i a chronic inflammatory yndrome that may reult in complication month to year after the onet of inflammation.
