Abstract :
PURPOE: To ae functional and anatomical outcome after foveal tranlocation with 360-degree retinotomy and imultaneou torional mucle urgery in patient with myopic neovacular maculopathy.
METHOD: Foveal tranlocation with 360-degree retinotomy wa performed in 11 eye of 11 patient with myopic neovacular maculopathy. Ten eye had imultaneou torional mucle urgery with receion of the uperior oblique mucle and tucking of the inferior oblique mucle. ilicone oil removal with or without intraocular len implantation wa performed 2 to 8 week after the primary procedure. Viual acuity, binocular function, and degree of cyclotorion were aeed preoperatively and potoperatively. Angle of retinal and globe rotation, ditance of foveal hift, and urgical complication were alo invetigated.
REULT: With a mean potoperative follow-up of 6.2 month (range, 3 to 13 month), viion improved (greater than 0.2 logarithm of minimal angle of reolution [logMAR] unit) in eight eye, wa unchanged in two eye, and worened (greater than 0.2 logMAR unit) in 1 eye. even of 11 eye (64%) had a final viual acuity of 20/50 or better. Five patient developed or maintained binocular fuion, four patient continued to have uppreion, and two patient developed diplopia that wa managed by pectacle with Frenel prim. ubjective cyclotorion wa le than 8 degree in 10 eye. Mean retinal and globe rotation were 23.4 degree and 19.8 degree, repectively. Average ize of the choroidal neovacular membrane wa 0.8 dik diameter, wherea the average ditance of foveal hift wa 1.5 dik diameter. After the primary procedure, three eye developed retinal detachment, one eye macular hole, and one eye proliferative vitreoretinopathy. Thee complication were uccefully managed by additional urgery.
CONCLUION: Foveal tranlocation with 360-degree retinotomy i effective in retoring viion in ome patient with myopic neovacular maculopathy. Although the development of torional diplopia i generally obviated by imultaneou extraocular mucle urgery, a relatively high incidence of urgical complication hould be taken into account with thi procedure.
