Abstract :
PURPOE: To ae quantitatively the cumulative effect of hyperglycemia on len tranparency in patient with juvenile type 1 diabete mellitu.
METHOD: ubject were 30 patient (30 eye) with type 1 diabete mellitu who had well-documented record on the duration of diabete mellitu and condition of glycemic control from the onet. They were 35 year of age or younger (mean, 26.0 year), had a hitory of type 1 diabete mellitu at leat 5 year (mean, 8.4 year), had corrected viual acuity of 20/20 or better, and howed no clinically apparent cataract on lit-lamp examination. Twenty-one eye of 21 ubject erved a age-matched normal control. They were 35 year of age or younger (mean, 25.7 year), had no diabete mellitu, had corrected viual acuity of 20/20 or better, and howed no ign of cataract on lit-lamp examination. The degree of len opacity wa quantified uing the anterior eye egment analyi ytem baed on the cheimpflug principle. An index wa created to repreent the cumulative effect of long-term glycemic control (hyperglycemic accumulation) by multiplying the average hemoglobin A1c value and the number of month from the onet.
REULT: The patient with diabete mellitu exhibited ignificantly greater degree of len opacity than the normal control (P = .017, Mann-Whitney U-tet). Among the patient with diabete mellitu, the len opacity wa greater in eye with retinopathy than thoe without retinopathy (P = .011). Multiple regreion analyi revealed that only the index of hyperglycemic accumulation ignificantly correlated with the degree of len opacity (P = .042).
CONCLUION: Accumulated effect of hyperglycemia i related to the len tranparency in patient with diabete.
