Intravitreal injection of crytalline cortione a adjunctive treatment of proliferative diabetic retinopathy

PURPOE: To report the clinical outcome and complication of intravitreal injection of crytalline cortione in patient undergoing par plana vitrectomy for treatment of proliferative diabetic retinopathy. METHOD: The propective, interventional cae erie tudy included 29 conecutive patient (29 eye) who underwent par plana vitrectomy for treatment of complicated proliferative diabetic retinopathy aociated with central retinal traction detachment. All patient received an intravitreal injection of 15 to 20 mg of crytalline triamcinolone acetonide at the end of urgery, and were operated on by the ame urgeon. Mean follow-up time wa 1.4 ± 1.1 month (median, 1 month; range, 0.30 to 4.9 month). REULT: At the end of the follow-up period, the retina wa attached in 26 of the 29 patient (89.7%). In three of the 29 patient (10.3%), a retinal redetachment had occurred. None of the patient developed iri neovacularization, and the iri neovacularization, preent preoperatively in 12 patient, lightly to markedly regreed in all 12 patient. Preoperative and potoperative intraocular preure value (P = .72) and blood glucoe meaurement did not vary ignificantly. A peudohypopyon coniting of cortione crytal in the inferior anterior chamber angle wa detected in one patient and reolved pontaneouly within 4 day. CONCLUION: The preent clinical tudy ugget that intravitreal injection of crytalline cortione with mot of the vehicle removed eem to be well tolerated by eye undergoing par plana vitrectomy for treatment of complicated diabetic proliferative retinopathy. In view of the antiphlogitic and antiproliferative effect of cortione, future randomized clinical trial may be indicated to invetigate further the role of intravitreal injection of crytalline cortione in the treatment of proliferative diabetic retinopathy.
